基础治疗慢性牙周炎对龈沟液的影响

Influence of Basal Treatment on Gingival Crevicular Fluid in Patients with Chronic Periodontitis

目的探讨基础治疗慢性牙周炎对龈沟液的影响。方法选取2008年12月—2009年12月慢性牙周炎10例,共入选28颗第一磨牙为观察牙进行基础治疗,观察基础治疗前、基础治疗后1个月、基础治疗后2个月患牙菌斑指数、牙周探诊深度、牙龈指数、牙周附着丧失和牙齿松动度的改善情况,及龈沟液量的变化。结果基础治疗后1个月和2个月菌斑指数、牙龈指数、牙周探诊深度、牙周附着丧失均低于基础治疗前,且基础治疗后2个月低于基础治疗后1个月(P<0.05)。基础治疗后2个月牙齿松动度较基础治疗前和基础治疗后1个月好转(P<0.05)。基础治疗后1个月和2个月龈沟液体积小于基础治疗前,且基础治疗后2个月小于基础治疗后1个月(P<0.05)。龈沟液体积与菌斑指数、牙龈指数、牙周探诊深度、牙周附着丧失、牙齿松动度呈正相关(P<0.05)。结论基础治疗后慢性牙周炎牙周组织炎症程度减轻,龈沟液量也随之减少。

Objective To explore influence of basal treatment on gingival crevicular fluid in patients with chronic periodontitis. Methods Ten patients with chronic periodontitis were enrolled, and twenty-eight first molars with basal therapy were observed. Dental plaque indexes, periodontal diagnosis depthes, gingival indexes, periodontal attachment loss, improvement of tooth mobility and changes of gingival crevicular fluid volume were observed before basal therapy, 1 month and 2 months after therapy. Results Dental plaque indexes, periodontal diagnosis depthes, gingival indexes and periodontal attachment loss 1 month and 2 months after basal therapy were significantly lower than those before therapy, and indexes 2 months after therapy were lower than those 1 month after therapy （P 〈 0. 05 ）. Improvement of tooth mobili- ty 2 months after therapy were significantly better than those before and 1 month after basal therapy （P 〈 0. 05）. Gingival crevicular fluid volumes 1 month and 2 months after therapy were significantly smaller than those before therapy, and volumes 2 months after therapy were also smaller than those 1 month after therapy （P 〈 0.05 ）. There were positive correlations between gingival crevicular fluid volumes and dental plaque indexes, periodontal diagnosis depthes, gingival indexes, periodontal attachment loss and improvement of tooth mobility（P 〈 0.05）. Conclusion Degrees of paradentium inflammation in patients with chronic periodontitis may be improved by basal treatment, and volume of gingival crevicular fluid may be also reduced.
