摘要
目的观察单核细胞趋化蛋白-1(MCP-1)、巨噬细胞(Mφ)在正常宫颈组织、宫颈上皮内瘤变及宫颈鳞癌组织中的表达,研究其与宫颈鳞癌临床病理参数的关系。方法采用SP免疫组化法检测20例正常宫颈组织,60例CIN,40例宫颈鳞癌组织中MCP-1和巨噬细胞的表达。采用SPSS14.0统计软件对结果进行统计学分析。结果 (1)与CINⅢ组相比,正常宫颈组MCP-1阳性评分升高(P<0.05),CINⅠ、CINⅡ、宫颈癌组中的MCP-1阳性评分升高(P<0.01);并且MCP-1阳性评分与宫颈鳞癌临床分期、浸润深度、血管转移、淋巴转移均呈正相关(P<0.01),但与分化呈负相关(P<0.01)。(2)与正常宫颈组比较,巨噬细胞在CINⅠ、CINⅡ中的表达降低(P<0.05),在CINⅢ中的表达明显降低(P<0.01)。与CINⅢ组相比,巨噬细胞在CINⅠ、CINⅡ、宫颈鳞癌组的表达明显升高(P<0.01)。与CINⅡ组相比,巨噬细胞在宫颈癌组的表达明显升高(P<0.01)。并且巨噬细胞与宫颈鳞癌临床分期、浸润深度、淋巴转移均呈正相关(P<0.01),与血管转移呈正相关(P<0.05)、但与分化呈负相关(P<0.01)。(3)在宫颈临床病理分期中,经Spearman相关性分析,MCP-1与巨噬细胞存在正相关(r=0.560,P=0.00)。结论MCP-1和巨噬细胞的异常表达与宫颈鳞癌的发生、发展相关。癌组织中浸润的MCP-1和巨噬细胞可以促进肿瘤血管、淋巴管的形成和肿瘤的浸润转移。
Objective To investigate the expression of monocyte chemoattractant protein - I (MCP - 1) and macrophage (Mop) in normal cervical tissue,cervical intraepithelial neoplasia (CIN) and squamous cervical carcinoma (SCC) and analyzed their differential ex- pression and possible role during progression to squamous cervical cancer. Methods The streptavidin peroxidase (SP) immunohistro- chemical staining was selected to detect the expression of MCP - 1 and Mφ in 20 cases of normal cervical squamous epithelial tissue ,60 cases of CIN tissue and 40 cases of SCC tissue. The results were analyzed with SPSS 14.0 in statistics. Results ( 1 ) Compared with CIN I, the expression score of MCP - 1 were higher in normal tissue ( P 〈 0. 05 ) and significantly higher in CIN Ⅰ , CIN II and SCC tissue (P 〈 0. 01 ). It exhibited a positive correlation with clinical stage of tumor, the depth of wall invasion and the metastasis of blood vessel and lymph nodes (P 〈 0. 01 ), a negative correlation with histology differentiation ( P 〈 0.01 ). (2) Compared with normal tissue, the macrophage density (MD) in the CIN I , CIN II were significantly lower obviously decrease (P 〈 0. 05 ) , were obviously decreased in CIN (P 〈 0.01 ). The MD in the CIN I , CIN II and SCC were obviously increased compared with GIN HI (P 〈 0.01 ). The MD were obviously increased compared with SCC (P 〈 0. 01). It exhibited a positive correlation to clinical stage of tumor, the metastasis of blood vessel and lymph nodes (P 〈0.01 ) and the depth of wall invasion (P 〈0.05), a negative correlation with histology differentiation (P 〈 0.01 ). (3) Through Spearman correlation analysis, a closely positive correlation was found between MCP - 1 and MD (r = 0. 550,P =O. 00). Conclusion The abnormal expression of MCP - 1 and Mop had relation with the progression to squamous cervical cancer. The in- filtrating MCP - 1 and Mop in the cervical cancer tissue promote the tumor angiogenesis, lymphatic vessel formation, invasion and metastat- ic spread.
