扩大胰十二指肠切除术治疗胰腺癌的研究进展

Advances in Extended Pancreatoduodenectomy for Carcinoma of Pancreas

胰腺癌作为一种恶性程度很高的消化道肿瘤,近年来发病率有明显升高的趋势,外科手术是目前唯一可能治愈的治疗手段。但胰腺癌起病多隐匿,发现时常已伴有淋巴结转移或邻近血管侵犯,因此手术切除率低,复发率高。国内外外科医师一直致力于改进术式以提高术后的远期疗效,在标准胰十二指肠切除术的基础上出现了以提高根治切除率为目的的扩大切除术。该文就扩大胰十二指肠切除术的病理生理基础、手术范围、适应证及手术难点等予以综述。

As a high level malignant gastric tumor, carcinoma of pancreas shows an obviously in- creasing morbidity and high mortality, and surgery is the only way to cure the disease at present. Howev- er the onset of carcinoma of pancreas is often insidious,and it is usually diagnosed with lymph node and vascular invasion when discovered. Therefore the resection rate is low with high recurrence. Surgeons from China and abroad have long been working on improving operations to better the long term out- comes. Extended pancreatoduodenectomy is invented on basis of traditional pancreatoduodenectomy for the purpose of improving radical excision rate. Here is to introduce the physiopathological basis,opera- tion scope, clinical indications and difficulty of extended oancreatoduudenectomv.
