化浊解毒方药结合针刺治疗慢性萎缩性胃炎40例临床观察

A Clinical Observation on Turbidity-Transforming Toxin-Resolving Formula Combined with Acupuncture for 40 Cases of Chronic Atrophic Gastritis

目的观察化浊解毒方结合针刺治疗慢性萎缩性胃炎(CAG)的临床疗效。方法选择CAG浊毒内蕴证患者120例,随机分为针刺组、中药组和针药组各40例,针刺组针刺中脘、天枢(双)、内关(双)等穴位,每日1次,5次为1个疗程,休息两日继续治疗。中药组给予化浊解毒方中药治疗,每日1剂,早晚两次温服。针药组口服化浊解毒方的同时给予针刺治疗,各组治疗时间均为6个月。观察治疗前后各组患者的胃痛、胃胀、嗳气、纳差、大便不爽等中医临床症状积分,同时于治疗后1、3、6个月时判定临床疗效。结果治疗1、3、6个月后针刺组与中药组总有效率比较,差异无统计学意义(P>0.05),治疗1、3、6个月后针药组与针刺组、中药组总有效率比较差异有统计学意义(P<0.01)。各组治疗后胃痛、胃胀、嗳气、纳差、大便不爽各临床症状积分均有明显改善,差异均有统计学意义(P<0.05);治疗后针药组各临床症状积分均明显优于针刺组、中药组,差异均有统计学意义(P<0.05)。结论化浊解毒方结合针刺可有效改善患者的临床症状,其近期疗效以及远期疗效均优于单纯的中药治疗和针刺治疗。

Objective To observe the therapeutic effect of turbidity-transforming toxin-resolving formula combined with acu- puncture on chronic atrophic gastritis （CAG）. Methods Totally 120 CAG patients with turbidity-toxin brewing internally pattern were randomized into the acupuncture group, medicine group and acupuncture combined with medicine group. The acu- puncture group was given acupuncture on Zhongwan （CV12）, bilateral Tianshu （ST25） and bilateral Neiguan （PC6） once daily, rested for 2 days and treated five times as one treatment course. The medicine group was given turbidity-transforming toxin- resolving formula one dose daily in the morning and evening. The acupuncture combined with medicine group was given acupuncture and turbidity-transforming toxin-resolving formula. The treatment lasted for 6 months. The TCM symptom scores including stomach pain, bloating, belch, poor appetite, uncomfortable stool and therapeutic effect were observed after 1, 3 and 6 months of treatment. Results There was no significant difference between the acupuncture group and medicine group in the efficacy after 1, 3 and 6 months of treatment （P〉0.05）. The efficacy of the acupuncture combined with medicine group was superior to that of the acupuncture group and medicine group after 1, 3 or 6 months of treatment （P〈0.01）. The symptom scores were significantly improved in all groups after treatment （P〈0.05）, especially in the acupuncture combined with medicine group （P〈0.05）. Conclusion Turbidity-transforming toxin-resolving formula combined with acupuncture for CAG can effectively relieve the symptoms with a better short-term and long-term efficacy as compared with medicine or acupuncture simply.
