电针治疗胃热炽盛型2型糖尿病患者35例临床观察 被引量:24

A Clinical Observation on Electro-Acupuncture for 35 Cases of Type 2 Diabetes Mellitus with Intense Stomach Heat Pattern

摘要 目的观察电针对胃热炽盛型2型糖尿病(T2DM)患者的临床疗效。方法 70例女性胃热炽盛型T2DM患者随机分为针刺组和电针组,每组35例,针刺组针刺脾俞(双侧)、胃俞(双侧)、大肠俞(双侧)等穴位,行平补平泻手法,每次留针30min;电针组在针刺组基础上使用电针仪用导线分别连接同侧上巨虚—内庭(双侧)、足三里—丰隆(双侧),频率为5Hz,每次治疗30min。两组均隔日治疗1次,治疗3个月。观察两组患者治疗前后空腹血糖(FPG)、空腹胰岛素(FINS)、胰岛素敏感性指数(ISI)、胰岛素抵抗指数(Homa-IR)、胰岛β细胞功能指数(Homa-β)及总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-c)、高密度脂蛋白胆固醇(HDL-c)水平。结果两组患者治疗后FPG、FINS、Homa-IR、TC、TG、LDL均较治疗前明显下降(P<0.01),ISI、HDL明显升高(P<0.01),且电针组治疗后Homa-β显著升高(P<0.01)。治疗后电针组FPG、FINS、ISI、Homa-IR的改善优于针刺组(P<0.05)。针刺组总有效率为82.86%,电针组总有效率为94.28%,两组比较差异无统计学意义(P>0.05)。结论针刺与电针对T2DM患者糖、脂质代谢及胰岛素抵抗具有良性调整作用,且电针在降低血糖、改善胰岛β细胞功能相关参数方面的作用优于针刺。 Objective To observe the therapeutic effect of electro-acupuncture (EA) on type 2 diabetes mellitus (T2DM) with intense stomach heat pattern. Methods Seventy female T2DM patients with intense stomach heat pattern were randomized into the acupuncture group and EA group, with 35 in each. The acupuncture group was given acupuncture on Pishu (BL20), Weishu (BL21) and Dachangshu (BL25) bilaterally with neutral supplementation and drainage and retained the needles for 30 minutes each time. The EA group was given EA on Shangjuxu (ST37)-Neiting (ST44) and Zusanli (ST36)- Fenglong (ST40) bilaterally at a frequency of 5Hz for 30 minutes. Both groups were treated once every other day. The fasting plasma glucose (FPG), fasting insulin (FINS), insulin sensitivity index (ISI), insulin resistance index (Homa-IR), β-cell function index (Homa-β), total cholesterol (TC), triglyceride (TG), low density lipoprotein (LDL-c), high density lipoprotein (HDL-e) were observed before and after 3-month treatment. Results The FPG, FINS, Homa-IR, TG, TC and LDL-c were significantly decreased but the ISI and HDL-c were significantly increased in both groups after treatment (P〈0.01). The Homa-β was significantly increased in the EA group (P〈0.01). The improvement of FPG, FINS, ISI and Homa-IR in the EA group was superior to that in the acupuncture group (P〈0.05). The total effective rate of the acupuncture group and EA group was 82. 86% and 94.28% respectively, with no significant difference between groups (P〈0.05). Conclusion Acupuncture and EA can regulate the glucose metabolism and lipid metabolism, relieve the insulin resistance and enhance the body 's sensitivity to insulin in T2DM. EA is more effective in lowering blood sugar and improving β-cell function parameters as compared with acupuncture.
