Alzheimer病的质子磁共振波谱分析

摘要 选择 2 4例Alzheimer病 (AD)病人 ,7例血管性痴呆病人和 2 5名健康老年人对照 ,用刺激回波采样方式 (STEAM )获取跨枕叶皮质区波谱 ,点分辨自旋回波波谱序列 (PRESS)获取右侧海马结构区波谱 ,体素大小2 0 .2mm× 2 0 .2mm× 2 0 .0mm ,分析N 乙酰天门冬氨酸 (NAA)、肌酐 (Cr)、胆碱 (Cho)、肌醇 (mI)及其与Cr的比值在三者间的差别。结果显示 ,枕叶皮质区和海马结构区的NAA、NAA/Cr在AD组和血管性痴呆组均比对照组降低有显著性意义 (P <0 .0 5) ;其余各代谢物的信号强度和比值差异均无显著性意义 (P >0 .0 5)。说明AD病人跨枕叶皮质区和海马结构区 1H -MRS的改变主要表现为NAA的降低 ,这种降低可以为AD的诊断提供参考信息 ,但是对痴呆不具有鉴别诊断意义。 To investigate the proton magnetic resonance spectroscopy ( 1H MRS) in the diagnosis and differential diagnosis of Alzheimer's Disease (AD). Stimulated echo acquisition mode (STEAM) in trans-occipital grey matter and point resolve spin echo spectroscopy (PRESS) in hippocampal formation region were used in 24 AD patients, 7 vascular dementia (VD) patients and 25 aged controls. Voxel size: 20.2mm×20.2mm×20.0mm. Metabolites of NAA, Cr, Cho, mI and their ratios to Cr were analyzed among the groups. RESULTS: Reduction of NAA and NAA/Cr were obvious in AD and VD groups where P <0.05 in both trans occipital grey matter and hippocampal formation, while the others were not. 1H MRS can provide reference information for the diagnosis of AD, which presents. NAA reduction in the regions of hippocampal formation and trans-occipital grey matter.The changes are the common manifestation of the aged brain and are not specific to AD.
