摘要 目的分析矢状窦旁脑膜瘤术后颅内血肿及严重脑水肿形成的原因,探讨手术中颅内静脉保护的临床意义。方法回顾性分析我院2008-05—2012-04收治的19例矢状窦旁脑膜瘤患者的临床资料。结果 1例患者术后头颅CT示术区瘤腔内出血,脑实质内血肿,行开颅血肿清除术;1例患者术后CT示严重脑水肿伴瘤腔内少量出血,经加强脱水治疗后改善;其余17例患者术后头颅CT提示不同程度脑水肿,经脱水治疗改善。结论术中颅内静脉损伤可能是矢状窦旁脑膜瘤术后出血及严重脑水肿的主要原因之一。精巧的显微手术技巧,术中妥善保护好静脉,避免损伤,可减少术后血肿及严重脑水肿的发生率。 Objective To analyze the cause of intracranial hematomas and serious brain edema after the surgery for pa- rasagittal meningioma, and to discuss the significance of intracranial veins protection during the operation. Methods The clini- cal data of 19 patients with parasagittal meningioma, from May 2008 to April 2012, were analyzed retrospectively. Results One patient showed intracranial hematomas in the tumorous cavity by the postoperative head CT, who underwent the cranioto- my to remove the bematomas. Serious cerebral edema and little bleeding in the tumorous cavity were found in another patient, who recovered after treatment of strong dehydration. In other 17 cases, postoperative brain CT showed edema in different de- gree, the dehydration treatment improved them. Conclusion The injury of intracranial veins during microsurgery could be one of main causes in the bleeding and serious cerebral edema after operation for parasagittal meningioma. The incidence of postop- erative intracranial hematomas and serious cerebral edema might be reduced by skillful microsurgical technique and good protec- tion of intracranial veins.
