微小RNA-181b在胃癌组织中的表达及其临床意义 被引量:4

Expression and clinical significance of microRNA-181b in gastric cancer

摘要 目的探讨微小RNA(miRNA)在胃癌组织中的表达及其临床意义。方法从52例福马林固定后石蜡包埋(FFPE)的胃癌组织样本中筛选鉴定出可以作为预后标志物的miRNA。从胃癌患者FFPE肿瘤组织样本和正常对照组织中分别提取出总RNA,应用实时定量逆转录聚合酶链反应(RT-qPCR)法检测候选miRNAs的表达水平,并结合临床资料分析评估其在晚期胃癌预后判断中的意义。结果miR-181b在胃癌组织中表达水平显著高于正常胃组织(P〈0.01)。多因素生存分析显示:miR-181b高表达胃癌患者总生存期显著低于miR-181b低表达患者,中位生存期分别为5.03(1.34-8.72)和8.36(7.18~9.55)个月。miR-181b低表达胃癌患者死亡风险HR为miR-181b高表达患者的0.164倍(P〈0.05)。二疗程后疗效与晚期胃癌患者总生存期明显相关。稳定(SD)、进展(PD)患者的死亡风险比(HR)分别为部分缓解(PR)患者的0.552倍(P〉0.05)和3.363倍(P〈0.05)。结论胃癌组织中存在miRNA的异常表达,其中miR-181b高表达与晚期胃癌不良预后显著相关。 Objective To investigate the expression and clinical significance of microRNA (miRNA) in gastric cancer. Methods The expression of candidate miRNAs was quantified from fifty-two advanced gastric cancer FFPE specimens. Total RNAs were extracted from normal and gastric tumor specimens. The levels of candidate miRNAs expression were quantified using real-time reverse transcriptase-polymerase chain reaction (RT-qPCR) analysis. In addition, the prognostic significance in advanced gastric cancer will be further evaluated by combining with clinical data analysis. Results The expression of miR-181b was significantly overexpressed in gastric tumors compared to normal gastric tissues (P 〈 0. 01 ). Kaplan-Meier survival analysis revealed that high level of miR-181b expression was closely associated with poor overall survival (OS). The median OS of miR-181b low expression patient subgroup was 8.36 (7. 18- 9. 55) months, while was only 5.03. (1.34-8.72) months for the high expression subgroup. Patients with low miR-181b expression showed a significant decrease in hazard ratio (HR) for death (HR =0. 164, P 〈 0. 05). The efficacy after two cycles was also found to be significantly correlated with the OS of advanced gastric cancer patients. Compared to patients with patial response (PR) , the HR for death were 0. 552 ( P 〉 0. 05 ) and 3. 363 ( P 〈 0. 05 ) for patients with stable disease (SD) and progressive diease (PD). Conclusion Abnormal expression of miRNAs has been implicated in gastric cancer, and high expression of miR-181b was closely associated with poor prognosis of advanced gastric cancer patients.
