N-乙酰半胱氨酸对大鼠心脏移植缺血再灌注损伤的保护作用 被引量:13

Protective effects of N-acetylcysteine on myocardial ischemia-reperfusion injury in the rat heart transplantation

摘要 目的观察N-乙酰半胱氨酸(NAC)预处理对心脏移植缺血再灌注损伤(IRI)的保护作用。方法健康雄性Lewis大鼠100只和BN大鼠20只,随机分为3组:对照组:摘取供体心脏前30min或移植前30min,经下腔静脉注射生理盐水2ml;供体预处理组:摘取供心前30min,经下腔静脉注射NAC300mg/kg,受体不处理;受体预处理组:受体于移植前30min,经下腔静脉注射NAC300mg/kg,供体不处理。供心冷藏于4℃组氨酸-色氨酸-酮戊二酸(HTK)液18h后建立大鼠腹腔异位心脏移植模型(Lewis→Lewis)。使用Kaplan—meier分析其生存率(BN→Lewis);分别于移植后6、24h取血测心肌酶学,再灌注24h取移植物检测超氧化物歧化酶(SOD)、脂质过氧化物(LPO)活性;光镜、电镜下观察心肌组织学及超微结构变化;免疫组织化学检测诱导型一氧化氮合酶(iNOS)、内皮型一氧化氮合酶(eNOS)蛋白表达;采用实时定量聚合酶链反应(Real—timePCR)检测iNOS、eNOSmRNA表达。结果大鼠生存率NAC预处理组明显高于对照组;移植后6hNAC供体预处理组肌酸激酶MB同工酶(CK—MB)、乳酸脱氢酶(LDH)较对照组低[(10681.11±2052.01)、(4019.00±678.03)U/L比(31093.11±6822.67)、(8197±807.20)U/L,P〈0.05],受体预处理组CK—MB、肌钙蛋白T(TnT)、LDH较对照组低[(12903.29±1789.46)U/L、(17.90±2.87)μg/L、(4235.17±585.88)U/L比(31093.11±6822.67)U/L、(49.51±6.41)ug/L、(8197.00±807.20)U/L。P〈0.05]。移植后24h供体预处理组TnT较对照组低[(11.08±1.53)μg/L比(20.28±2.90)μg/L,P〈0.05],受体预处理组TnT、LDH较对照组低[(7.06±0.71)μg/L、(774.00±113.13)U/L比(20.28±2.90)μg/L、(1619.50±289.11)U/L,P〈0.05];移植后24h,LPO活性较对照组降低[(9.48±1.05)/(7.82±1.56)比(10.90±2.28)μmol/L],SOD活性增高[(4.71±1.09)/(4.57±1.09)比(2.38±0.39)U/ml,P〈0.05]。心肌组织学及超微结构的损伤程度显示NAC预处理组较对照组明显减轻;iNOS蛋白表达和mRNA表达供体预处理组和受体预处理组较对照组减低[(1.50±0.22)/(1.63±0.26)比(3.00±0.15),(0.95±0.41)/(0.43±0.17)比(1.00±0.41)];eNOS蛋白表达和mRNA表达供体预处理组和受体预处理组较对照组升高[(3.60±0.16)/(3.40±0.26)比(2.00±0.21),(3.06±1.47)/(1.48±0.53)比(1.00±0.65),P〈0.05]。结论在大鼠心脏移植中NAC对供心IRI具有保护作用,其主要是通过直接抗氧化及提高氧化酶活性来减少心肌细胞损伤,从而减轻心肌IRI。 Objective To explore the protective effect of N-acetylcysteine (NAC) on graft ischemia-reperfusion injury (IRI) in the rat heart transplantation. Methods 100 healthy male Lewis rats and 20 healthy male BN rats were randomly divided into four groups. In c, ontrol group, 2 ml normal saline was infused via the inferior vena cava 30 rain before donor harvest or implantation. In donor preconditioning group, NAC (300 mg/kg body weight, iv. ) was infused via the inferior vena cava 30 min before donor harvest, and the recipient was not processed.
