颅内压监测下脑室外引流、腰大池引流治疗脑室岀血的疗效观察 被引量:6

摘要 目的探讨颅内压监测下脑室外引流、腰大池引流治疗脑室岀血的疗效。方法以本院就诊的重型脑室出血患者为研究对象。随机分成试验组和对照组。试验组给予颅内压监测下行脑室外引流;腰大池引流,对照组根据患者症状、体征和CT扫描结果进行脑室外引流、腰大池引流。比较两组预后、并发症(颅内感染、脑积水、脑疝、再出血)、脑室引流时间、住院时间的差别。结果①试验组疗效显著优于对照组,差异有统计学意义(χ2=9.621,P<0.05);②试验组颅内感染、脑积水和脑疝发生率显著低于对照组,差异有统计学意义(P<0.05);③试验组脑室引流时间和住院时间均显著低于对照组,差异有统计学意义(P<0.05)。结论颅内压监测下脑室外引流、腰大池引流治疗脑室岀血疗效好,可减少并发症,降低住院时间。 Objective To explore the effect of brain ventricle drainage and lumbar cistern drainage by monitoring intracranial pressure in treatment of intraventricular hemorrhage. Methods 40 patients with severe intraventricular hemorrhage were treated with brain ventricle drainage and lumbar cistern drainage by monitoring intracranial pressure. They were divided into the observe group. The other 40 patients treated with brain ventricle drainage and lumbar cistern drainage without monitoring intracranial pressure were divided into the control group. The prognosis, complications (intracranial infection, hydrocephalus, cerebral hernia, bleeding), ventricular drainage time and hospitalization time were compared between the two groups. Results The clinical efficacy of the observe group was significantly better than that of the control group ( P&lt;0.05); The rate of intracranial infection, hydrocephalus and cerebral hernia in the observe group was significantly lower than that in the control group ( P&lt;0.05); The time of ventricular drainage and hospital stay in the observe group was significantly less than that in the control group ( P&lt;0.05). Conclusions Brain ventricle drainage and lumbar cistern drainage with monitoring intracranial pressure had a better effect in treatment of intraventricular hemorrhage.
