磁共振弥散成像对宫颈癌表观扩散系数的研究 被引量:10

摘要 目的探讨MR弥散成像对宫颈病变的诊断价值。方法回顾性分析15例正常宫颈结构及36例宫颈疾病患者(病理证实20例宫颈癌、16例宫颈良性病变)的弥散成像表观扩散系数值对比研究,总结宫颈病变的DWI特点。结果正常宫颈内膜、结合带及肌层三层结构平均ADC值为(1.63±0.24)X10-3mm2/s,结合带与肌层的ADC值差异有统计学意义(P<0.05);而结合带与内膜及内膜与肌层的ADC值差异无统计学意义(P>0.05)。20例宫颈癌患者平均ADC值为(0.93±0.17)X10-3mm2/s,显著低于正常宫颈的内膜、结合带和肌层三层结构的平均ADC值(P<0.05);宫颈良性病变平均ADC值为(1.77±0.23)X10-3mm2/s,与宫颈癌ADC值差异有统计学意义(P<0.05)。结论宫颈癌在DWI图像及ADC图上具有特征性,DWI能够区分正常宫颈组织及宫颈良性病变,对宫颈癌有重要的诊断价值。 Objective To explore diagnose value of diffusion weighted imaging in cervi- cal lesion. Methods 15 female patients without cervical disease and totally 36 cases with pathologically diagnosed (20 female patients with cervical carcinoma and 16 fe- male patients with benign lesion of cervix) were included in this study. Result The mean ADC value of normal cervix tissue was ( 1.63 ± 0.24) X 10-3mm2/s, ADC value was statistically different between conjunctive zone and muscular layer of cervix (P〉0.05). The mean ADC value of cervical carcinoma lesions was (0.93±0.17) X 10-3mm2/s, the ADC value was significantly higher than that of normal cervix tissue (P〈0.05). The mean ADC value of benign lesion of cervix was (1.77 ±0.23)X10-3mm2/s, there were statistic differences between benign lesion and cancerous tissue in the uterine cervix (P 〈0.05). Conclusion DWI and ADC measurement has a potential ability to distinguish carcinoma from normal tissue and benign lesions of the uterine cervix. It is clinically useful in diagnosing cervical carcinoma.
