内镜窄带成像筛查食管病变的应用分析 被引量:1

摘要 目的观察内镜窄带成像(NBI)筛查食管病变的作用。方法对400例患者进行研究,同一病例自身对照,在El服局部麻醉药物或无痛麻醉状态下,先以常规内镜模式进镜,进镜时详细观察食管,记录发现的可疑病灶,退镜至贲门时,用NBI模式退镜再次观察食管,记录发现的可疑病灶,于可疑病灶处取活检行病理检查。结果NBI与常规内镜图像相比,两者对Barrett食管鳞柱状上皮交界处病变轮廓的显示差异有统计学意义(P〈0.01);NBI对高级别上皮内瘤变及低级别上皮内瘤变的检出率均明显优于常规内镜[96.2%(25/26)比76.9%(20/26)、91.7%(22/24)比62.5%(15/24)],差异有统计学意义(P〈0.05)。结论NBI操作简便,对病变轮廓显示清晰,能进一步提高常规内镜对早期食管癌及癌前病变的检出率,在食管疾病的筛查中具有重要的实用价值。
