老年COPD患者血清GSH与IL-18的表达研究

摘要 目的:本文通过对临床老年COPD患者与健康对照组血清GSH水平和IL-18水平的表达差异及相关性研究,探讨以GSH为代表的氧化应激保护作用和以IL-18为代表的炎症因子在COPD发生过程中的作用和机制。方法:老年COPD患者32例,健康对照组30例,通过比色法和酶联免疫法测定血清中的GSH和IL-18水平,比较各组间GSH和IL-18表达差异及其相关性。结果:①急性COPD患者经常规治疗后转入稳定期时,各项肺功能指标(FEV1.0和FEV1.0/FVC)、GSH水平、IL-18水平均比急性加重期有明显改善;具体体现为COPD稳定期较急性加重期FEV1.0和FEV1.0/FVC指标显著的升高(P<0.05),GSH水平有明显的升高(P<0.05),IL-18水平有显著的降低(P<0.05);②对照组较COPD患者急性加重期和稳定期各组指标均有显著差异(P<0.05),但是GSH水平在稳定期则无差异(P>0.05)。③COPD患者急性加重期时GSH水平较低且IL-18水平很高,COPD患者稳定期时GSH水平较高,IL-18水平较低,正常组则是GSH水平较高且IL-18水平很低,但是各组IL-18的水平和GSH水平之间的却没有显著的相关性。结论:这反映了在常规COPD治疗过程中消炎药物的应用能很快降低炎症因子表达水平,但提高GSH水平却并非和降低IL-18水平是同时同步起作用的,这可能和个体差异有关,同时也反映了当前药物治疗COPD的单一性,而非系统性的从各个层面治疗COPD。 The expressing differ- ence and the relevance of GSH and II, - 18 for the gerontal patients with COPD and the controls is studied. The effects and mecha-nisms in GSH representing oxidative stress protection and IL - 18 representing iniqam- matory factor are explored during the gener- ating process of COPD. Methods :The colori- metric method and the enzyme - linked im- munosorbent assay( ELISA )method are em- ployed to assay GSH and IL- 18 in serum, and the difference and relativity in GSH and IL- 18 are analyzed. The following results are obtained: ①When the acute patients with COPD turn into the stable conditions, the lung function indices (FEVI. 0 and FEV1.0/FVC) ,GSH and IL - 18 level im- prove apparently. The lung function indices in COPD stable condition increase in terms of those in COPD acute exacerbations. The level of GSH enhances while IL - 18 re- duces. ②There exist apparent differences in all indices except for GSH between the pa- tients in COPD acute exacerbations and the stable conditions and the controls. ③When the patients with COPD are in the acute ex- acerbations,GSH is relatively low,and IL - 18 is high. As for the patients in the stable condition, the situation was reverse. As a reference,GSH is high and IL- 18 is low for the controls. However, there was no sig- nificant correlation between IL - 18 and GSH in 'all groups, Conclusion: Which shows that the application of anti - inflam- matory drug during the treatment of COPD can decrease the expressing level of inflam- matoly factor but enhance GSH. But the in- crease in GSH is not synchronous with re- duction in IL- 18,which indicates that is related to the individual difference and sire- plicity of treatment for COPD
