幽门螺旋杆菌感染与胃癌及胃黏膜疾病的相关分析

Analysis of the relationship between gastric cancer and gastric mucosal disease with Helicobacter pylori infection

目的探讨幽门螺旋杆菌(Hp)感染与胃癌及胃黏膜疾病发生的关系;方法经胃镜及病理检查确诊的胃癌患者47例作为观察组,经胃镜及病理检查确诊为慢性浅表性胃炎、慢性萎缩性胃炎、胃溃疡、消化性溃疡共161例患者作为对照组。两组患者进行14C尿素呼气试验检测Hp,同时观察组47例胃癌患者胃镜分型按照Borrmann分类为Ⅰ、Ⅱ、Ⅲ、Ⅳ型,组织学按照Lauren分为肠型和弥漫型,按照位置分为贲门型和非贲门型,观察不同部位和分类中的Hp感染情况。结果慢性浅表性胃炎的Hp感染阳性率最低,胃溃疡Hp感染阳性率最高,慢性萎缩性胃炎Hp感染阳性率第二,四组病人经统计学分析,χ2=6.988,P<0.05,存在显著性差异;胃癌Hp感染中非贲门与贲门感染相比较差异显著(72.7%vs 42.8%)。结论慢性萎缩性胃炎、胃癌的发生与Hp感染有一定的关联,非贲门位置的胃癌的发生率要显著高于贲门位置。

【Objective】 Reaserch Hp infected to the relationship between the occurrence of gastric cancer and gastric mucosal disease.【Methods】The observation group 47 cases gastric cancer by gastroscope and pathological wxamination diagnose,and the Control group 161 case atrophic gastritis and gastric ulcer by gastroscope and pathological wxamination diagnose.Two groups pations be in progress 14C urea breath test and test Hp,and The observation group 47 cases somatotype into 1,2,3,4type by Borrmann mould and hyphology Lauren mould somatotype into visible pattern and gstric pattern and and according the position of divided into cardia and non cardia to observe the effects of different parts of and classification.【Resluts】Chronic superficial gastritis HP infection positive rate was the lowest of gastric ulcer HP infection positive rate was highest,chronic atrophic gastritis HP positive rate of infection is second,χ2=6.988,P＆lt;0.05;The non cardia gastric cancer compare with cardia gastric cancer are obviously statistical significance（72.7% vs 42.8%）.【Conclusion】The atrophic gastritis and gastric cancer occurrence have relationship with infect Hp,And non cardia gastric cancer occurrence higher than cardia gastric cancer.
