26例原发性胆囊癌手术治疗的临床体会

Experience of Diagnosis and Surgical Treatment on 26 Cases of Primary Gallbladder Carcinoma

目的：探讨胆囊癌的诊治经验。方法：回顾性分析了我院1990~2010年收治26例胆囊癌患者的临床资料。结果：术前诊断率46.1%,69.2%胆囊癌合并胆囊结石,B超、CT是诊断胆囊癌的主要手段。1年生存率34.6%,5年生存率11.5%。结论：I期胆囊癌行单纯胆囊切除术,II、III、IV期胆囊癌首选根治性手术,V期胆囊癌采取综合治疗。早期诊断且行根治性手术是提高胆囊癌患者生存的有效方法。对合并胆囊结石的高危患者宜积极行胆囊切除术。

Objective：To explore the experience of diagnosis and surgical treatment on primary gallbladder carcinoma.Methods：The clinical data of 26 cases of primary gallbladder carcinoma from 1990 to 2000 were collected and analyzed retrospectively.Results：The preoperative diagnosis rate was 46.1%.69.2% of gallbladder cancer patients contained gallbladder stones.The 1-,5-year overall survival rates of gallbladder cancer patients were 34.6% and 11.5%,respectively.Ultrasonography（USG）,and contrast computed tomography（CT） are the main methods of diagnosis for gallbladder cancer.Conclusion： Gallbladder cancer patients of Phase I were treated with simple cholecystectomy.Phase II,III,IV gallbladder cancer patients were treated with radical surgery.Phase V gallbladder cancer patients were treated with comprehensive treatment methods.Early diagnosis and radical surgery is an effective method to improve the survival of gallbladder carcinoma patients.Patients with gallbladder stones are at high risk and should be treated with simple cholecystectomy agrresively.
