丙泊酚复合芬太尼静脉麻醉在无痛阴道取卵术中的应用

Application of Intravenous Combined Anesthesia with Propofol and Fentanyl in Painless Trans- vaginal Ooeyte Retrieval

【目的】探讨丙泊酚联合芬太尼静脉麻醉下经阴道超声引导穿刺取卵术的麻醉及其取卵效果和对体外受精一胚胎移植（IVF-ET）结局的影响。【方法】将IVF．ET患者分为麻醉组及对照组，前者在上述麻醉下取卵，后者肌注地西泮和哌替啶后取卵。观察两组麻醉效果、取卵效果及IVF-ET的结局。【结果】麻醉组麻醉效果及手术时间均优于对照组，其差异有显著性（P〈0．05），且无明显不良反应。麻醉组获卵率较对照组高（P〈0．05），但受精率、卵裂率、优胚率、胚胎种植率及妊娠率与对照组比较差异无显著性（P〉0．05）。【结论】在丙泊酚联合芬太尼静脉麻醉下经阴道超声引导穿刺取卵可获得满意的临床效果，并且对IVF-ET的结局无不良影响。

[Objective]To explore the effect of intravenous anesthesia with propofol and fentanyl in trans- vaginal oocyte retrieval on the outcome of pregnancy in vitro fertilization and embryo transfer （IVF-ET）. [Methods] The patients with IVF-ET were divided into anesthesia group and control group. The anesthesia group underwent oocyte retrieval under the above anesthesia, and the control group underwent oocyte retrieval after the injection with diazepam and pethidine. The anesthesia and oocyte retrieval efficacy and the outcome of IVF-ET in two groups were observed. [Results]The anesthesia effect and operation time in anesthesia group were better than the control group, and there was significant difference（ P d0.05）, and no adverse reaction occurred. The oocyte retrieval rate in anesthesia group there was no significant difference in fertilization rate, was higher than that in control group（ P d0.05）, but cleavage rate, good embryo rate, embryo implantation rate and pregnancy rate between two groups（ P 〉0.05）. [Conclusion]Intravenous combined anesthesia with propofol and fentanyl in transvaginal oocyte retrieval can achieve satisfactory efficacy, and has no effect on the outcome of IVF-ET.
