不同抗甲状腺药物系统治疗Graves病的对比观察 被引量:24

摘要 目的比较在Graves病治疗中使用丙基硫氧嘧啶和甲巯咪唑的有效性和安全性。方法选取80例确诊Graves病患者,分为丙基硫氧嘧啶组和甲巯咪唑组,以滴定法治疗,起始剂量分别为丙基硫氧嘧啶300 mg/d,甲巯咪唑30 mg/d。随诊观察2年。观察指标为甲状腺功能(FT3、FT4、TSH)、甲状腺抗体(TRAb、TpoAb)、肝功能(AST、ALT、TBIL)、血常规。结果甲巯咪唑组较丙基硫氧嘧啶组症状控制所需的时间短[(6.6±3.8)周vs(9.4±3.5)周],TSH恢复正常快[(7.2±4.3)个月vs(10.7±5.2)个月],依从性好,但两组缓解率差异无统计学意义(22%vs 17%)。两组治疗前后TRAb都有显著下降,但两组间比较TRAb下降幅度差异无统计学意义[(16.9±7.5)U/L vs(14.6±6.1)U/L]。治疗中肝功能比较,丙基硫氧嘧啶组ALT(68±10.2)U/L、AST(64±9.5)U/L,均高于甲巯咪唑组的ALT(36±7.5)U/L、AST(32±6.8)U/L,甲巯咪唑组TBIL较丙基硫氧嘧啶组升高,但尚无临床意义。结论甲巯咪唑控制Graves病症状快,依从性好,维持稳定,肝脏不良反应较小,可以作为非特殊情况下药物治疗甲状腺功能亢进症的首选。但甲巯咪唑和丙基硫氧嘧啶治疗Graves病的缓解率类似,对TRAb的调节效果无明显差异。
