耳针治疗颈型颈椎病的瞬即时疗效评价

Transient Effect of Auricular Acupuncture for Cervical Type of Cervical Spondylosis

目的:评价耳针治疗颈型颈椎病的瞬即时疗效。方法:采用随机对照单盲试验研究方法,评价耳针治疗颈型颈椎病的瞬即时疗效。患者来源:2008年11月—2009年7月上海市针灸经络研究所门诊就诊的96例颈型颈椎病患者。分组:采用SPSS 17.0统计软件编程的计算机随机分组,其中耳针组48例,假耳穴组48例。观察时间:治疗后15 min、30min、60 min。干预耳穴:耳针组采用颈椎穴、枕穴、神门穴耳针干预;假耳穴组采用相对应的3个假耳穴耳针干预。主要结局指标:压痛阈、耐痛阈、VAS、颈椎活动度。安全性指标:不良事件发生率。结果:耳针组与假耳穴组相比,治疗前与颈型颈椎病预后相关的各基线资料之间没有统计学差异。治疗后30 min和60 min耳针组的VAS评分优于假耳穴组;治疗后15 min、30 min和60 min耳针组的压痛阈评分优于假耳穴组;治疗后15 min、30 min时耳针组的颈椎左右旋转活动度评分优于假耳穴组;假耳穴组有2.1%的患者出现了耳针干预侧耳廓及颈部的异常疼痛,耳针移除后自然缓解。结论:耳针治疗颈型颈椎病的随机对照单盲试验表明耳针能够安全且有效地瞬即时减轻颈型颈椎病患者的疼痛程度,改善颈椎左右旋转活动度。

Objective：To evaluate the transient effect of auricular acupuncture on cervical type of cervical spondylosis. Meth-ods：We evaluated the transient effect of auricular acupuncture for cervical type of cervical spondylosis by a randomized controlled single-blind clinical trial. Subjects source： November 2008 to July 2009,96 patients with cervical type of cervical spondylosis from Shanghai Institute of Acupuncture and Tuina Science clinic. Group： Forty-eight patients were assigned randomly in auricular acu- puncture group, and 48 patients in sham auricular points by the SPSS 17.0 computer program. The outcomes were observed at 15 nfin, 30 min ,60 min after treatment. Intervention： auricular acupuncture points included cervical point, cushion and Shenmen, and three sham acupuncture points of the helix ipsilateral to the site of neck pain were used for the invasive needle control. Main out- come measures： tenderness threshold, pain tolerance threshold, visual analogue score, cervical range of motion. Safety indicators： the incidence of adverse events. Results ： No significant differences were noted in any baseline characteristics between the two groups. At 30 min and 60 min after treatment, the VAS scores of auricular acupuncture group were better than those in sham auric- ular points group;at 15 min,30 min and 60 min after treatment, the pain threshold score of auricular acupuncture group was better than that in sham auricular group;when treatment for 15 min,30 min, the auricular acupuncture could significantly increase the cervical motion range to left and right, compared with the fake auricular acupuncture group. There was 2.1 percentage subjects in sham auricular points group suffered abnormal pain at the intervention site of ear and neck and eased after removal of the acu- puncture. Conclusion：Auricular acupuncture treatment has transient analgesic effect, can reduce neck pain of patients who catch cervical tvoe of cervical soondvlosis, and improve the activity of cervical rotation around safely.
