摘要
为探讨低位直肠癌经肛门局部切除术的疗效与可行性，回顾分析2001年1月至2005年1月收治的35例低位直肠癌经肛门局部切除的患者资料，观察手术并发症，并对患者进行随访，观察其预后。结果显示，（1）分化良好型腺癌（高分化腺癌、中分化腺癌）28例，分化不良型腺癌（低分化腺癌、印戒细胞癌、黏液腺癌）7例；伴有脉管瘤栓3例，无脉管瘤栓32例。（2）T0期8例，T1期16例，T2期6例，T3期5例。（3）随访5年以上，复发3例，均行Miles术；5年生存率为91．43％（32／35）。结果表明，只要严格把握手术适应症，术后结合放、化疗，低位直肠癌经肛门局部切除可获得良好疗效。
In order to explore the therapeutic effect and feasibility of transanal excision for low rectal carci- noma authors restrospectively analyzed the data of 35 eases received this procedure from Jan. 2001 yr. to Jan. 2005 yr. which included operative complication,and prognosis observed through follow-up. As results, 1）fine differentiated adenocarcinoma（high differentiated one,and middle differentiated one） was in 28 ca- ses;poor differentiated adenocarcinoma （low differentiated one, signet-ring cell carcinoma, mucinous one） was in 7 cases;rectal carcinoma associated with vasal tumor embolus in 3 cases, without vasal tumor embo- lus in 32 cases. 2）8 cases were at To stage,16 cases at T1 stage,6 cases at T2 stage,5 cases at T3 stage. 3） the follow-up of more than 5 years found 3 cases suffered from recurrence and followed by performing Mi- les procedure on them,the survival rate of 5-year was 91. 43%（32/35）. Above results show that as long as strictly mastering operative indication, and combining with postoeprative radiochemotherapy, above proce- dure could obtain better therapeutic effect for treating low rectal carcinoma.
