摘要 研究旨在通过对不同保存期内全血理化指标的检测,评价全血质量的变化。选择30例健康献血员采集全血2 U/人,每袋全血采用无菌方式分为6份,每份血袋内装60—80 mL全血,置于储血冰箱(4±2)℃条件下保存。分别在全血保存的0 d、7 d、14 d、21 d、28 d和35 d后取样,检测血液pH值、血常规、电解质(K+、Na+)、游离血红蛋白(FHb)、Na+-K+-ATP酶、2,3-二磷酸甘油酸(2,3-DPG)、葡萄糖、乳酸和血流变。全血中K+含量在7 d内迅速升高,在保存期21 d的浓度为(22.46±6.88)mmol/L,有些已达不到现行国家标准[1](GB 18469—2001:全血保养液ACD—B时,钾离子浓度≤21 mmol/L);Na+离子浓度逐渐降低,在保存的7 d含量为(136.33±5.98)mmol/L,亦未达到国家标准(GB 18469—2001:全血保养液ACD—B时,钠离子浓度≥146 mmol/L);pH值在保存期28 d内能达到国家标准(GB 18469—2001:全血保养液ACD—B时,pH为6.6~7.0),但0 d和21 d比较有显著性差异(P<0.01);Na+-K+-ATP酶的活性0 d和14 d比较有显著性差异(P<0.05);2,3-DPG的含量7 d发生明显变化(P<0.05);游离血红蛋白在保存21 d能达到国家标准(GB 18469—2001:全血保养液ACD—B时,血浆血红蛋白≤0.29 g/L);HCT在保存期35 d内无显著性变化(P<0.05);全血保存21 d后高切黏度显著升高(P<0.05),7 d后低切黏度的升高也有显著性(P<0.05)。说明全血随着保存时间的延长,其理化指标变化有所不同,研究为血液的保存和临床科学合理输血提供理论数据。 To evaluate the whole blood quality through the study of the physiological and biochemistry proper- ties and the functions of the whole blood during storage, uints whole blood were collected from 30 healthy donors, and each bag of the whole blood were divided into six bags of 60--80 mL, sterilizedly. The blood routine test, he- morhedogie properties, activity levels of Na + -K+ -ATP enzyme, concentrations of Na + ,K + ,LD,Glu and 2,3-DPG of the whole blood were measured. The K + concentration increased rapidly within 7 d, and it wasn' t meet current national standards after 21d (the concentration was (22. 46±6. 88) mmol/L) ; the Na+ concentration wasn't meet the national standards neither was decreased to ( 136.33± 5.98 ) mmol / L in 7 d. The pH was different be- tween the 0 d and 21 d significantly(P 〈0. 01 ), and it wasn't meet the national standards after 28 d. During the storage of 0 d and 14 d, the activity levels of Na + -K + -ATP was a significant difference between the 0 d and 7 d of the enzyme was decreased significantly ( P 〈 0. 05 ). There concentration of 2,3-DPG. The FHb wasn't meet cur- rent national standards after 28 d. There was no significant change of HCT during storage time. The high shear rates was different between the 0 d and 21 d significantly(P 〈 0. 05 ). The low shear rates was increased significantly after 7 d (P 〈 0. 05). It is conclused That physical and biochemistry indicators change differently time, therefore, provide theoretical data for blood preservation and clinical scientific and rational blood transfusion.
