Silk ligation of laparoscopic varicocele surgery by two trocars

目的探讨两孔法腹腔镜精索静脉丝线结扎术治疗原发性精索静脉曲张症（PV）患者疗效.方法2007年1月至2011年12月采用两孔法腹腔镜精索静脉丝线结扎术对275例PV患者进行治疗.其中双侧57例,左侧218例.结果275例PV患者手术均获成功.平均手术时间6~20 min,平均住院时间2.2d（1~3d）;随访l^12个月,无睾丸萎缩及术后复发.有10例出现睾丸鞘膜积液,通过保守治疗获得治愈.结论初步证实两孔法腹腔镜精索静脉丝线结扎术治疗PV患者安全有效.但是长期疗效仍需要大规模、多中心、随机、对照研究加以证实.

Objective To investigate the efficacy of silk ligation of laparoscopic surgery by two trocars in treatment of primary varicocele（PV）. Methods Two hundred and seventy-five PV patients from January 2007 to December 2011 in our hospital, including 57 PV patients with bilateral sides and 218 cases with left unilateral side, were treated by silk ligation of laparoscopic surgery by two trocars. Results The operations were successfully finished for all the patients. The mean operation time was 6~20 min. The mean hospital time was 2.2 days. During the follow-up of 1 to 12 months, no testicular atrophy or PV recurrence were observed. 10 cases were found to have hydrocele, and were cured by conservative therapy. Conclusion The silk high ligation of laparoscopic surgery by two trocars was preliminarily found ot be safe and effective for treatment of PV patients. The long term of efficacy and side effects need to be further confirmed by large scale, multicentre, randomized, controlled trials.
