眼内异物摘出术后视网膜脱离

Retinal detachment after intraocular foreign bodies removal with vitrectomy.

目的 报告并分析眼内异物摘出术后视网膜脱离发生率及有关病因。方法 眼内异物 2 0例 (2 0眼 ) ,用玻璃体切除术摘出眼内异物。结果 2 0眼中术后视网膜脱离 4眼 (2 0 % )。术后视力提高 14眼 (70 % ) ,视力不变 3眼 (15 % ) ,视力下降 3眼(15 % )。结论 眼内异物摘出术后视网膜脱离发生原因 :(1)残留玻璃体形成牵拉 ;(2 )激光封闭裂孔不确切 ;(3)术中未发现的小裂孔 ;(4)术前玻璃体积血浑浊未能及时激光治疗 ;(5 )异物较大 ,从玻璃体切除的切口摘异物时对周边部视网膜划伤。

To report the incidence of retinal detachment after intraocular foreign bodies removal with vitrectomy,and to analyze the relative reasons.Methods A retrospective analysis of 20 consecutive patients (20 eyes) who underwent pars plana vitrectomy for the treatment of intraocular foreign bodies removal was conducted.The standardized three port vitrectomy was used in all injured eyes.Results Four (20%) of 20 eyes had retinal detachment.Visual acuity increased in 14 eyes (70%),stabilized in 3 eyes (15%),and deteriorated in 3 eyes (15%).Conclusion The possible reasons of retinal detachment after intraocular foreign bodies removal with vitrectomy included:(1) the remnant vitreous traction formation;(2) not proper photocoagulation around the retinal hole;(3) tiny retinal hole not found during the surgery;(4) no preoperative photocoagulation on the surface of retina around the foreign body;and (5) the peripheral retinal tear induced by large foreign body removal.
