腹腔镜肾癌根治术围术期机体应激状态指标变化研究

Changes of body stress indexes of patients undergoing laparoscopic radical nephrectomy for renal carcinoma during perioperative period

目的:探讨腹腔镜肾癌根治术患者围术期的机体应激状态指标变化情况。方法:选取2010年7月～2012年9月于本院进行腹腔镜肾癌根治术的32例患者为观察组,同期进行开放性手术的32例患者为对照组,后将两组患者术前及术后1d、3d的血清皮质醇、肾上腺素、血糖及血压、心率进行检测及比较。结果:术前两组患者的血清皮质醇、肾上腺素、血糖及血压、心率均比较,P均>0.05,均无显著性差异,而观察组手术后1d及3d的上述几项指标则均明显地低于对照组,P均<0.05,均有显著性差异。结论:腹腔镜肾癌根治术患者围术期的机体应激状态指标波动幅度明显小于开放性手术,故认为腹腔镜肾癌根治术对机体的不良影响更小。

Objective： To investigate the changes of body stress indexes of patients undergoing laparoscopic radical nephrectomy for renal carcinoma during perioperative period.Methods：A total of 32 patients underwent laparoscopic radical nephrectomy for renal carcinoma in our hospital from July 2010 to September 2012 were selected as observation group,and 32 patients underwent open operation during the same period were selected as control group,then the serum cortisol,epinephrine,blood glucose and blood pressure,heart rate of two groups before and after the operation and 1d,3d after the operation were detected and compared.Results：Before undergoing the operation,the serum cortisol,epinephrine,blood glucose and blood pressure,heart rate of two groups were not significantly differenct（P all 0.05）,but these indexes of observation group were lower than those of control group after the operation 1 and 3 days after undergoing the operation（P all0.05）.Conclusion：The fluctuation range of body stress indexes of patients underwent laparoscopic radical nephrectomy for renal carcinoma during perioperative period are smaller than those of open operation,the adverse influence of laparoscopic radical nephrectomy for renal carcinoma is smaller.
