臀上动脉穿支皮瓣修复骶尾部巨大褥疮 被引量:1

摘要 目的探讨臀上动脉穿支皮瓣的手术方法和临床疗效。方法对骶尾部巨大褥疮先行清创和封闭负压引流(VSD)技术进行治疗。再用单侧或双侧臀上动脉穿支皮瓣移位修复。自2007年1月以来临床应用12例,切取皮瓣面积为22cm×12cm~14cm×7cm。结果 12例皮瓣全部存活,随访6个月~2年,穿支皮瓣存活良好,无复发。结论彻底清创和VSD技术控制骶尾部褥疮感染后,为后期臀上动脉穿支皮瓣的修复创造了良好条件。臀上动脉穿支皮瓣具有操作简便、皮瓣血运良好、供区直接缝合、并发症少、成功率高,可一期完成修复。是骶尾部褥疮治疗较为理想的方法。 Objective To investigate the superior gIuteal artery perforator flap surgical methods and clinical efficacy. Methods sacral tail huge bedsore anticipatory debridement and closed negative pressure drainage (VSD) technology for treatment. Then the unilateral or bilateral superior gluteal artery perforator flap for repairing, Since the 2007 years since January 12 cases of clinical application,cutting flap area of 22crux 12cm to 14crux 7cm. Results 12 cases of skin flap all live, and were followed up for 6 months to 2 years,wear a flap survival is good,no recurrence. Conclusion Thorough debridement and VSD technical control sacral tail bedsore infection, for later on in a hip artery flap repair created good condition,hip artery flap perforators with simple operation, good flap blood supply for direct suture zone, fewer complications,and high success rate, a complete repair period. Is sacral tail bedsore therapy is an ideal method.
