NSE与Cyfra21-1水平变化对诊断肺癌种类的相关性研究 被引量:2

摘要 目的探讨血清神经元烯醇化酶(NSE),细胞角蛋白19片段抗原(Cyfra21-1)对小细胞肺癌与非小细胞肺癌辅助诊断意义。方法收集80例小细胞肺癌患者,74例非小细胞肺癌患者,140例其他非肺肿瘤的肺部疾病患者及50例健康体检者对照组,采用电化学发光法检测各例血清中标志物NSE和Cyfra21-1的浓度,并比较分析各组患者中NSE和Cyfra21-1阳性的百分率,以及两者同时阳性的百分率。结果三组疾病患者的NSE阳性率分别依次为60.0%,21.6%,22.9%,差异有统计学意义(P<0.05);Cyfra21-1阳性率分别依次为35.0%,59.4%,28.6%,差异有统计学意义(P<0.05);NSE和Cyfra21-1两者同时阳性时,小细胞肺癌以NSE升高为主,平均升高12.3倍,非小细胞肺癌以Cyfra21-1升高为主,平均升高26.4倍。结论 NSE和Cyfra21-1可能分别为小细胞肺癌及非小细胞肺癌的较特异血清肿瘤标志物。两者同时阳性时,测定值明显升高者具相对应的区分肺癌种类诊断意义。 Objective To investigate the significance of detection of serum level of NSE and Cyfra21-1 in diagnosis of small cell and non-small cell lung cancer.Methods The serum sample of 80 patients with small cell lung cancer and 74 patients with non-small cell lung cancer,140 patients with lung disease without lung cancer and 50 healthy persons as control were collected and the levels of NSE and Cyfra21-1(tumor marker)were determined by electrogenerated chemiluminiscence method.The percentage of single marker and double markers in each group were compared.Results The positive rates of NSE,in small lung cancer,non-small lung cancer and patients with other lung diseases were 0.0%,21.6%,22.9%,and that of Cyfra21-1 were 35.0%,59.4%,28.6%,respectively,all showed significant differences between groups(P 0.05);The level of NSE was increased in those with small cell lung cance(r averageed 12.3 times)and the level of Cyfra21-1 was increased in patients with non-small cell lung cancer(averaged 26.4 times)when both NSE and Cyfra21-1 were positive.Conclusions NSE and Cyfra21-1 can be respectively used as small cell lung cancer and non small cell lung cancer marker.Combined detection of NES and Cyfra21-1,may be useful for the diagnosis of different types of lung cancer.
