改良经闭孔尿道中段无张力悬吊术治疗女性压力性尿失禁38例

Curative effect of tension-free vaginal tape-obdurate technique on women with stress urinary incontinence： analysis of 38 cases

目的探讨改良经闭孔尿道中段无张力悬吊术（TVT-O）治疗女性压力性尿失禁的临床效果。方法回顾性分析运用改良TVT-O治疗38例女性压力性尿失禁患者的一般情况、手术时间、出血量、并发症、住院时间及中短期疗效。结果患者平均手术时间、术中出血和住院时间分别（32．2±1．7）min、（20．5±2．8）ml和（3．5±0．7）d，术后患者尿失禁症状消失。无膀胱穿孔、耻骨后血肿及切口感染等并发症发生。总体短期有效率为97．4％，1年后有效率为94．7％。结论TVT-O治疗女性压力性尿失禁中短期疗效良好，手术安全性高、操作简单易学。

Objective To assess the curative effect of tension-free vaginal tape-obdurate technique （TVT-O） on women with stress urinary incontinence. Methods We retrospectively analyzed general conditions, time consumed during operation, amount of bleeding, complications, days of hospital stay, and short-term curative effect of 38 cases of female patients with stress incontinence treated with the improved TVT-O from July 2006 to March 2010. Results All operations were performed under local anesthesia. The average length of time consumed during operation was 32.2 ± 1.7 min, the amount of blood loss was 20. 5 ± 2.8 ml, and the average hospital stay was 3. 5 ± 0.7 d. No severe complications such as bladder perforation, hematoma, or incision infection occurred, and incontinence symptoms in all patients disappeared after operation. Conclusion TVT-O is a good, safe, and convenient operation for women with stress urinary incontinence.
