胆囊息肉样病变的彩色多普勒超声诊断

Diagnostic Value of Ultrasound in Polypoid Lesions of Gallbladder

目的探讨彩色多普勒超声对胆囊息肉样病变的诊断价值。方法分析经手术病理证实的97例胆囊息肉样病变的彩色多普勒超声声像图特征,包括二维声像图特征及彩色多普勒特征。结果胆囊息肉常为多发、有蒂、乳头状高回声结节。胆囊腺肌增生常为基底较宽的低回声结节。胆囊腺瘤常为单发,大多有蒂,中或高回声结节。胆囊癌常为基底较宽、中或低回声结节。恶性病变组彩色多普勒超声显示血流信号丰富,阻力指数明显高于良性病变组。结论彩色多普勒超声对胆囊息肉样病变良恶性的鉴别具有一定的临床价值。

Objective To evaluate the value of color Doppler flow imaging in diagnosis of polypoid lesion of gallbladder. Method Analyzed the sonographic features of 2-dimensional ultrasonography and CDFI of 97 patients with polypoid lesion of gallbladder proved by operation and pathology. Result Polys appeared as multiple,pedunculated, hypereehoic lesions on ultrasonographty. Adenomyomatoses appeared as widely basialis, hypoechoie lesions. Adenomas appeared as solitary, peduneulated, middle or hyperechoic lesions. Adenocarcinomas appeared as solitary, widely basialis, hypoechoic lesions. The color signals in malignant lessions were more than those in begion lessions, and resistive index （RI） in malignant lessions was much higher than that in benign lessions. Conclusion Color Doppler sonography has important clinical value in diagnosis of polypoid lesion of gallbladder.
