2型糖尿病合并冠心病中医证候的调查研究

Questionnaire of Traditional Chinese Medicine Syndrome on Type 2 Diabetes Mellitus Combined with Coronary Heart Disease

目的:探讨2型糖尿病(T2DM)合并冠心病(CHD)的证候分型。方法:以调查问卷的形式搜集患者信息,采用因子分析和聚类分析对其证候进行初步的探索性分析。结果:①患者的BWI:27.58±3.90,WC:95.40±11.18cm,WHR:1.06±0.11。②在T2DM合并CHD与未合并组的比较中,在年龄上(P=0.001)及合并有高血压(P=0.001)、周围神经病变(P=0.000)、视网膜病变(P=0.008)的比率上有差异。③将T2DM合并CHD聚为五类。结论:①T2DM人多数肥胖。②T2DM合并CHD中老年人易患,且更易合并有高血压病、周围神经病变和视网膜病变等并发症。③T2DM合并CHD可分为阴阳两虚,血瘀水停型;气虚血瘀型;气阴两虚型;痰湿血瘀型和脾虚湿盛型。

Objective： To investigate the Tradition Chinese Medicine syndrome differentiation of type 2 diabetes mellitus （TZDM） combined with coronary heart disease （CHD）. Methods： The study used a questionnaire of a prospective and clinical epidemiology survey methodology, used the Factor Analysis and Cluster Analysis to make a preliminary exploratory analysis of the syndrome differentiation on the patients of T2DM combined with CHD. Results： 1. The mean observed： BWI： 27. 58 ＋3.90, WC： 95.40~11.18cm and WHR： 1.06＋0. llcm. 2. On the comparison of T2DM combined with CHD and not combined, in the aspect of the age, P=0.001, and the combination of hypertension, P = 0.001, DNP, P = 0.000, and retinopathy, P = 0. 008 were obviously different. 3. T2DM combined CHD could be eventually clustered into five categories. Conclusion ： 1. Most patients of T2DM are fat. 2. The elderly are susceptible to T2DM combined CHD and are more easily combined with compli- cations of hypertension, DPN, and retinopathy. 3. They can be categorized into five： Yin and Yang deficiency, pattern of blood stasis causing water retention; Qi deficiency and blood stasis ; Qi and Yin deficiency; phlegm-dampness and blood stasis ; spleen deficiency and excessive dampness.
