腹腔镜与内镜下逆行胆胰管造影联用在胆囊取石治疗中的作用

目的:分析研究腹腔镜胆囊切除术(LC)联合术中内镜下逆行胆胰管造影(ERCP)及乳头括约肌切开(EST)取石一期治疗胆囊结石合并胆总管结石的可行性。方法:将68例行LC联合ERCP治疗的胆囊结石合并胆总管结石患者设为观察组,选择同期行传统开腹胆囊切除、胆总管探查取石T管引流治疗的60例患者为对照组,比较记录两组的临床治疗情况。结果:全部患者均手术成功,取石成功率均为100%。与对照组比较,观察组手术时间较长,但术后通气时间和术中出血量及住院时间较少,并发症发生率低于对照组,两组比较差异有统计学意义(P<0.05)。结论:LC联合术中ERCP一期治疗胆囊结石合并胆总管结石有较好的临床疗效,有利于患者的恢复,且不良反应少。

Objective:Laparoscopic and endoscopic retrograde cholangiopancreatography associated with the role in the treatment of gallbladder stone.Methods:68 cases of gallbladder stones with common bile duct stones treated with LC combined with ERCP were selected as observation group,another 60 patients of the same period treated with conventional open cholecystectomy,common bile duct exploration T-tube drainage were selected as the controal group,analysis and comparison on clinical treatment of the two groups was done.Results:Operations of the two groups were all successful,stone extraction rate both 100%.Conclusion:LC combined with intraoperative ERCP a treat gallstones and biliary stones better clinical outcome.
