头颅磁共振质子波谱分析在海马硬化诊断中的应用价值 被引量:2

摘要 目的评价头颅磁共振质子波谱(。H—MRS)在海马硬化诊断中的应用价值。方法对20例海马硬化患者进行1H—MRS检查,测定N-乙酰天门冬氨酸(NAA)、肌酸复合物(Cr)、胆碱复合物(Cho)的峰值,计算NAA/Cr+Cho、NAA/Cr、Cho/Cr比值,并与对照组(同期健康体检者和有鼻窦、头皮等部位病变但无脑内实质病变的非癫痫者)(18例)进行比较。结果在海马硬化组病侧和对侧的NAA/(Cr+Cho)比值均低于对照组[(0.57±0.03)、(0.64±0.02)比(0.78±0.02)],病侧与对侧比较差异有统计学意义(P〈0.01)。NAA/Cr比值病侧与对侧均低于对照组[(1.09±0.67)、(1.38±0.27)比(1.62±0.01)],差异有统计学意义(P〈0.01)。Cho/Cr比值病侧高于对照组[(1.29±0.45)比(1.15±0.04)],差异有统计学意义(P〈0.01),但病侧与对侧[(1.29±0.4)比(1.31±0.07)]相比,差异无统计学意义(P〉0.05)。结论‘H—MRS能够早期发现海马硬化,NAA/(Cr+Cho)值可作为诊断早期海马硬化的主要参数。1H-MRS和MRI相互补充可提高对海马硬化诊断的敏感性。
