股骨顺向髓内钉大转子进钉点研究进展 被引量:3

摘要 股骨近端外偏角型顺向髓内钉治疗股骨干骨折和股骨近侧干骺端骨折已广泛应用于临床。进钉点位置选择是该髓内钉插钉技术的要点之一,既要保证骨折生物力学稳定性,又要尽可能避免软组织损伤,减少对术后髋关节功能的影响。股骨近端大转子形状不规则,邻近解剖结构较为复杂。与从梨状窝、粗隆窝等处进钉相比,从大转子顶点进钉具有手术操作时间短、术中透视次数少、术中失血少、软组织损伤小等优势,但仍会造成一定程度的臀中肌止点损伤;从大转子顶点附近不同位置进钉,对骨折生物力学稳定性及操作亦有不同影响。该文就股骨顺向髓内钉大转子进钉点研究进展作一综述。
