摘要
目的:探讨紫杉醇联合顺铂方案同步放疗治疗局部晚期鼻咽癌的临床疗效及不良反应。方法:32例局部晚期鼻咽癌患者,应用紫杉醇120mg/m2,d1、顺铂25mg/m2,d1-3,分别于放疗第1天、第28天给药。放疗结束后第4、7周继续给予紫杉醇135mg/m2,d1、顺铂25mg/m2,d1-3方案辅助化疗。放疗采用Varian直线加速器6MV X射线照射鼻咽及颈部,鼻咽剂量DT 70-76Gy,2Gy/次,共35-38次,颈部剂量DT 60-70Gy,2Gy/次,共30-35次。结果:所有患者均完成放疗,其中22例患者放疗后完成2次化疗,10例患者放疗后完成1次化疗。近期疗效(治疗结束后1个月复查):有效率78.1%,其中CR 2例,PR 23例。远期疗效:3年总生存率为84.4%,鼻咽及颈部3年局部控制率分别为93.8%和96.9%,远处转移率为21.9%。结论:紫杉醇联合顺铂方案同步放疗治疗局部晚期鼻咽癌有助于提高生存率,减少局部复发和远处转移。
Objective： To evaluate the efficacy and toxicity of paclitaxel and cisplatin in patients in concomitant radiotherapy for locally advanced nasopharyngeal carcinoma. Methods： Thirty - two patients with locally advanced nasopharyngeal carcinoma were enrolled. On day 1 and day 28 during radiotherapy,patients were scheduled to receive two courses of concomitant chemotherapy by using paclitaxel 120mg/m2,dl plus cisplatin 25mg/m2, d1-3 Adjuvant chemotherapy was paclitaxel 135mg/m2, dI plus cisplatin 25mg/m2, d1-3 on weeks 4,7 after radiotherapy. Radiotherapy was performed by using Varian linear accelerator,6 MV X - ray. The dose of nasopharyngeal field was DT 70 - 76Gy/ 35 -38f,while the dose of neck field was DT 60 -70Gy/30 -35f. Results：All patients completed radiotherapy, but for concomitant ehemoradiotherapy ,22 of the 31 patients completed 2 cycles of chemotherapy ,while the other 10 could only acceptted 1 cycle due to various reasons. The total effective rate was 78.1% with CR 2 eases and PR 23 cases. The 3 -year overall survival rate was 84.4% ,the local eontral rates of nasopharyngeal field and neck field at 3 years were 93.8% and 96.9% ,respectively. While the distant metastasis rate at 3 years was 21.9%. Conclusion：Paclitaxel with cisplatin as concurrent chemoradiotherapy for locally advanced nasopharyngeal carcinoma can improve survival rate and decrease the rates of regional recurrence and metastasis.
