摘要
目的:探讨血管内皮细胞生长因子受体-2(VEGFR-2)及微血管密度(MVD)在大肠癌组织中的表达与大肠癌生物学行为之间的关系。方法:采用免疫组化SP方法,检测54例大肠癌组织、18例癌旁正常组织中VEGFR-2的表达,以CD34标记血管内皮细胞,对CD34阳性血管进行MVD计数。结果:VEGFR-2在大肠癌组织中的阳性表达率为59.26%,在癌旁正常组织中阳性表达率为22.22%,差异有统计学意义(P<0.05)。VEGFR-2的表达与大肠癌浸润深度、淋巴结转移、远处转移、临床分期呈正相关(P<0.05)。大肠癌组织中VEGFR-2阴性表达者5年总生存期明显高于VEGFR-2阳性表达组(分别为48.97%及20.64%,P<0.05)。大肠癌组织、癌旁正常组织中MVD分别为(28.57±9.41个/HP、10.53±3.94个/HP),二者比较差异有统计学意义(P<0.01)。MVD与浸润深度、淋巴结转移、远处转移及临床分期呈正相关(P<0.01)。MVD≤28个/HP的胃癌患者总生存期较MVD>28个/HP患者明显延长(分别为63.63%及28.57%),差异有统计学意义(P<0.01)。经相关分析,VEGFR-2阳性表达率与MVD呈正相关(r=0.463,P<0.01)。结论:VEGFR-2、MVD可作为大肠癌发生侵袭转移和判断预后的指标。
Objective:To investigate the expression of vascular endothelial growth factor receptor -2 (VEGFR - 2) and coretlation of microvessel density(MVD) in colorectal carcinoma and their significance in carcinogenesis progression, invasion and metestasis of colorectal cancer. Methods: The expression of VEGFR - 2 and MVD were studied by immunohistochemistry in 54 colorectal carcinoma specimens and 18 neighboring non - cancerous normal tissues ,20 patients with colorectal benign lesions. The MVD was assayed by CD34 positive blood vessels counting. Results:The positive rates of VEGFR - 2 were 59.26% , 22.22% , respectively, in colorectal carcinoma and neighboring non - cancerous normal tissues, patients with colorectal benign lesions ( all P 〈 0.05 ). The positive expression rates of VEGFR - 2 were significantly correlated with depth of invasion,lymph node metastasis, distant metastasis and clinical stage in colorectal carcinoma patients( P 〈 0.05 ). The five - year overall survival was lower in patients with tumors positive for VEGFR -2 than in those with tumors negative for VEGFR -2(48.97% vs 30.64% ). The MVD score in colorectal carcinoma was significantly higher than in neighboring non - cancerous normal tissues ,patients with colorec- tal benign lesions( P 〈 0.01,28.57 ±9.41number/HP and 10.53± 3.94number/HP). The MVD in colorectal cancer was significantly associated with depth of invasion,lymph node metastasis, distant metastasis and clinical stage ( P 〈 0.01 ). The five - year overall survival was lower in patients with tumors having high MVD score than in patients with tumors having low MVD score(63.63% vs 28.57% P 〈 0.01 ). In colorectal carcinoma,expression had a positive correlation with VEGFR - 2 and MVD ( r = 0. 463, P 〈 0.01 ). Conclusion: VEGFR - 2 and MVD are related to the growth,infiltration, metastasis and prognosis of colorectal cancer, that can be used as useful markers to guide the choice of therapy and predicts prognosis in colorectal cancer.
