不同剂量布地奈德联合沙丁胺醇雾化吸入治疗婴幼儿喘息的疗效比较

The effects of different doses of budesonide with albuterol aerosol inhalation in treatment of wheezing infants

目的:比较不同剂量布地奈德混悬液联合沙丁胺醇溶液雾化吸入治疗婴幼儿喘息的疗效。方法:将102例符合婴幼儿喘息诊断的患儿随机分为3组。在综合治疗的基础上,Ⅰ组(高剂量联合用药组)用布地奈德混悬液2 mL、沙丁胺醇溶液0.5 mL、生理盐水2 mL雾化吸入;Ⅱ组(低剂量联合用药组)用布地奈德1 mL、沙丁胺醇溶液0.5 mL、生理盐水2 mL雾化吸入;Ⅲ组(对照组)用沙丁胺醇溶液0.5 mL、生理盐水3 mL雾化吸入。3组所用空气压缩雾化吸入机、用法、疗程均相同。观察比较3组疗效和不良反应。结果:3组疗效在显效率方面比较,差异有统计学意义(χ2=23.46,P<0.05),Ⅰ组优于Ⅱ组、Ⅲ组,Ⅱ组优于Ⅲ组;3组在咳嗽消失、喘息缓解、哮鸣音消失时间及平均住院天数方面比较,差异有统计学意义(P<0.05),且Ⅰ组优于Ⅱ组、Ⅲ组,Ⅱ组优于Ⅲ组(P<0.05)。未发现不良反应。结论:布地奈德混悬液联合沙丁胺醇溶液雾化吸入治疗婴幼儿喘息有显著疗效,且高剂量疗效优于低剂量,值得临床推广应用。

Objective：Compare efficacy of different doses of budesonide combined with salbutamol solution aerosol inhalation treat- ment of wheezing infants. Methods ： 102 cases who are wheezing infants were divided into three groups randomly. On the basis of com- prehensive treatment,the I group （high-dose combination therapy group） mixed budesonide 2ml,salbutamol solution 0.5ml while nor- mal saline 2 ml aerosol inhalation;group IT （low-dose combination therapy group） ： selection of budesonide Germany lml, salbutamol solution 0.5rnl and normal saline 2 ml atomization inhalation; II （control group） ： selection of salbutamol solution 0.5ml and normal saline 3 ml inhalation. The three groups used the same compressed air aerosol inhalation machine,usage and course of treatment. Ob- serve the three groups and compare the efficacy and adverse reactions. Results：The effective rate and differences in the three groups of efficacy are statistically significant （X2 =23.46,P 〈0.05）. Group I is superior to group II and group III ,while group U is superior to group III ;The vanish of cough,wheezing remission,the vanish of wheeze time and the average length of stays in hospital among the three groups are statistically significant （P 〈 0.05）. Group I is superior to group II and group BI ,while group II is superior to group m （P 〈 0.05 ）. No adverse reactions are found. Conclusion ： Budesonide combined with salbutamol solution aerosol inhalation therapy in wheezing infants showed efficacy, and efficacy of high dose is superior to low dose, which deserves clinical application.
