微型钛板内固定与颌间牵引固定治疗下颌骨单发性骨折术后下颌中线位移改变的研究

Comparison between rigid internal fixation with minimal titanium plate and intermaxiilary traction and fixation on the influence of mandibular midline shift in patients with mandibular fracture

目的探讨微型钛板内固定与颌问牵引固定治疗下颌骨单发性骨折术后对于下颌中线位移改变的影响。方法将106例下颌骨单发性骨折的患者分成2组，其中微型钛板内固定治疗组（A组）57例，颌间牵引固定治疗组（B组）49例。术后第2、7天测量每例患者下颌中线至上颌中线的水平距离，取2次测量平均值为术后值（a值）；经术后1、2年回访，重新测量，取两次测量的平均值为回访值（b值）；统计学检验每例患者的回访值与术后值之间的差值（b—a）。结果A组b—a差值小于B组（P〈0．05）。结论与颌间牵引相比，微型钛板内固定治疗下颌骨单发性骨折，其术后下颌中线位移改变较小。

Objective To introduce a contrast study between rigid internal fixation （RIF） with minimal titanium plate and intermaxillary traction and fixation on the influence of mandibular midline shift in patients with mandibular fracture. Methods Totally 106 patients with mandibular fracture were divided into 2 groups： RIF with minimal titanium plate in 57 cases （ Group A）, and intermaxillary traction and fixation in 49 cases （Group B）. The horizontal distance between of mandibular midline and maxillary midline was measured at 2nd and 7th day postoperatively, and their mean value was the postoperative value （Value a）. Follow-up observation was performed at the 1 st and 2nd year, and the distance between the two midlines was measured. The mean value of the follow-up measurement was Value b. The value b - a was studied by statistical evaluation. Results The value b - a in Group A was significant smaller than that in Group B （ P〈 0.05）. Conclusion Comparison to in- termaxillary traction and fixation, RIF with minimal titanium plate is more suitable for the treatment of mandibular fracture with less deflexion degree of the mandibular midline.
