小细胞肺癌组织CD56、CgA、Syn及TTF-1表达及联合诊断意义 被引量:6

摘要 目的研究神经细胞黏附分子(CD56)、嗜铬蛋白A(CgA)、突触素(Syn)与甲状腺转录因子-1蛋白(TTF-1)在小细胞肺癌(SCLC)中的表达,探讨其表达在SCLC中的意义及联合诊断价值。方法应用免疫组化SP法,检测CD56、CgA、Syn及TTF-1在62例SCLC及40例非小细胞肺癌(NSCLC)(包括腺癌22例,鳞癌12例,大细胞癌6例)组织中的表达,并对表达结果进行分析。结果 SCLC组织中CD56、CgA、Syn及TTF-1的阳性表达率分别为93.5%、9.7%、87.1%、80.6%;NSCLC组织中CD56、CgA、Syn及TTF-1的阳性表达率分别为7.5%、0、15.0%、47.5%。CD56、Syn及TTF-1在SCLC与NSCLC组织中的阳性表达率比较差异均有显著性(χ2=12.21~74.89,P<0.01);CgA在两者中的阳性表达率比较差异无显著性(P>0.05)。SCLC中4项指标的表达在不同病理学参数间差异均无显著性(P>0.05)。结论 CD56、CgA、Syn及TTF-1在SCLC与NSCLC组织中的表达存在差异,其联合检测对SCLC的诊断有重要价值。 Objective To study the expressions of neural cell adhesion molecule(CD56),chromogranin(CgA),synaptophysin(Syn),thyroid transcription factor-1(TTF-1) in small cell lung cancer(SCLC) and assess their value in combined diagnosis.Methods Immunohistochemistry SP was used to detect the expressions of CD56,CgA,Syn and TTF-1 in 62 patients with SCLC,and 40 with non-small cell lung cancer(NSCLC,including 22 adenocarcinoma,12 squamous cell carcinoma and six large cell carcinoma),the results were analyzed.Results The positive expression rates of CD56,CgA,Syn and TTF-1 in SCLC were 93.5%,9.7%,87.1%,and 80.6%,respectively,and that in NSCLC were 7.5%,0,15.0%,and 47.5%.The expressions of CD56,Syn and TTF-1 in SCLC were significantly higher than that in NSCLC,the difference was significant(χ2=12.21-74.89,P<0.01);and that of CgA was not(P>0.05).In SCLC,the positive rates of the four markers were not significantly different among different pathological parameters(P>0.05).Conclusion The expressions of CD56,CgA,Syn and TTF-1 are different between SCLC and NSCLC.A combined detection of them is of great value in the diagnosis of SCLC.
