抗CCP抗体、抗Sa抗体和类风湿因子在早期类风湿关节炎中的诊断价值 被引量:4

摘要 目的研究抗环瓜氨酸肽抗体(抗CCP抗体)、抗Sa抗体与类风湿因子(RF)在早期类风湿关节炎(RA)诊断中的意义。方法收集2011年1月至12月在解放军四五二医院住院的RA患者151例及同期在该医院做健康体检的健康人员93例的血清标本,分别检测其抗CCP抗体、抗Sa抗体和RF的水平,并对检测数据进行统计分析,探讨其诊断RA的灵敏度及特异度。结果抗CCP抗体+抗Sa抗体+RF三项联合检测的灵敏度及特异度分别与抗CCP抗体、抗Sa抗体以及RF单项检测比较,差异均具有统计学意义(P<0.05);病程<1年的RA患者,其抗CCP抗体+抗Sa抗体+RF联合检测的灵敏度为95.6%;病程≥1年的灵敏度为90.4%,两者比较差异有统计学意义(P<0.05)。结论抗CCP抗体、抗Sa抗体和RF是早期诊断RA较好的指标,三者联合检测能提高检测灵敏度及诊断率,适用于早期RA的筛查诊断。 Objective To study the clinical significance of anti-CCP antibody,anti-Sa antibody and RF in early rheumatoid arthritis (RA) diagnosis. Methods The serum samples of 151 patients with rheumatoid arthritis,93 cases of the healthy controls from PLA 452 Hospital during Jan. 2011 and Dec. 2011 were collected and then detected anti-CCP antibody, anti-Sa antibody and RF among them, and statistical analysis was carried out to explore their sensitivity and specificity for diangonsing RA. Results There were significant differences between anti-CCP + anti-Sa + RF combined detection and anti-CCP, anti-Sa, RF single detection in detection sensitivity and specificity ( P 〈 0.05 ). The sensitivity of combined detection of RA patients with disease course of 〈 1 year was 95.6% and with disease course of ≥1 year was 90.4% ,and there was significant differences ( P 〈 0.05 ). Conclusion Anti-CCP, anti-Sa and RF are good indexes for early RA diagnosis, combination of the three is better for improving its sensitiyity and diagnosis rate, which is applicable for the early screening of RA.
