杭州市1099例性高危人群性传播疾病流行病学分析

Epidemiologic Analysis of Sexually Transmitted Diseases in High Risk Group in Hangzhou City

目的了解2008年1月—2011年12月杭州市卖淫嫖娼拘留人员的性传播疾病的感染情况。方法对2008年1月—2011年12月派出所拘留的卖淫嫖娼人员进行性病艾滋病的实验室检查,并收集相关资料作流行病学分析。结果在1 099例卖淫嫖娼人员中共检出性病199例,总检出率18.11%,其中以梅毒感染最多,达到10.19%。4年内卖淫组梅毒感染74例,嫖娼组感染38例,2组间差异有统计学意义(P<0.01)。4年内卖淫组淋球菌感染25例,嫖娼组感染5例,2组间差异有统计学意义(P<0.01)。2008年卖淫嫖娼2组之间梅毒感染差异有统计学意义,卖淫组显著高于嫖娼组。2009年和2010年2组淋球菌感染差异有统计学意义,卖淫组均高于嫖娼组。沙眼衣原体和人类免疫缺陷病毒(HIV)感染在卖淫组和嫖娼组间差异无统计学意义。结论卖淫嫖娼人员是性病的重要传染源,加强对这类人群的性病流行病学监测是预防和控制性病流行的重要措施之一。

Objective To investigate the epidemic trends of sexually transmitted diseases（STDs） among prostitutes and whoring goers in Hangzhou city from January 2008 to December 2011.Methods Venereal laboratory examination was carried out in police detaining prostitutes and whoring goers from January 2008 to December 2011,and the data were collected and analyzed.Results Among 1 099 prostitutes and whoring goers in detention,199 persons were detected to have STDs,the overall STDs detection rate was 18.11%,and syphilis infection was the most and its detection rate was 10.19%.There were 74 cases of syphilis infection in prostitution and 34 cases in whoring group in four years,and the difference between two groups was significant（P〈0.01）.There were 25 cases of Neisseria gonorrhoeae infection in prostitution and five cases in whoring group in four years,and the difference between two groups was significant（P〈0.01）.Among two groups,the rate of syphilis infection in prostitution group was significantly higher than that in whoring group in 2008.And there was significant difference in Neisseria gonorrhoeae infection between prostitution group and whoring group both in 2009 and in 2010.There was no significant difference of chlamydia trachomatis and HIV infection between prostitution group and whoring group.Conclusion Prostitutes and whoring goers are the important infection source of STDs,and strengthening the epidemiological surveillance could play a great role in preventing and controlling the spread of STDs.
