中药治疗痤疮疗效及对免疫功能调节作用研究

Curative Effect of Traditional Chinese Medicine in Treating Acne Vulgaris and Immune Function of Regulation

目的探讨中医辨证治疗痤疮的临床疗效及其对免疫功能的调节。方法将106例痤疮患者随机分为中医辨证治疗组和对照组。治疗组53例给予中医辨证分型治疗;对照组53例给予罗红霉素150 mg/次,2次/d,异维A酸胶丸10 mg/次,2次/d,外用维A酸乳膏,每晚1次。用痤疮皮损计数法观察治疗前后皮损情况,评估中医辨证治疗的疗效和对症状反复的影响;测定患者治疗前后和健康者血清白细胞介素(IL)-2、IL-4、IL-10和干拢素(INF)-γ水平,观察其变化。结果治疗完成后2组总有效率差异无统计学意义(86.79%和84.91%,P>0.05);中医治疗组对深在炎性丘疹、脓疱、结节、囊肿等的疗效优于对照组(P<0.001),复发率显著低于对照组;治疗前,2组患者IL-2、INF-γ均低于健康者,IL-4、IL-10高于健康者,治疗后2组血清IL-4、IL-10水平均有下降,2组间比较差异有统计学意义(P<0.01),2组血清IL-2、INF-γ水平均升高,2组间比较差异有统计学意义(P<0.01)。结论中医辨证治疗痤疮安全有效,复发率低,且对机体免疫功能有调节作用。

Objective To explore the effect of traditional Chinese medicine （TCM） on ache vulgaris and immune function of regulation. Methods 106 patients with ache vulgaris were randomly divided into two groups. The treatment group （53 cases） were treated with TCM and the control group （53 cases） were treated with Roxithtomycin tablets 150 mg, twice a day, Isotretinoin soft capsules 10 mg, twice a day, plus external use of Tretinoin cream. Condition of skin lesions and their changes were assessed with ache lesion counting method before and after treatment in order to evaluate the efficacy of TCM treatment and its implication on ache relapse. The levels of serum IL-2, IL-4, IL-10 and INF-γwere measured before and after therapy in all patients and healthy people. Results There was no significant difference between TCM group and control group in total effective rate （P〉0.05）. TCM group showed better effect on controlling deep inflammatory papules/pustules and nodules/cysts/ abscesses （P〈0.001）. Follow-up results after treatment revealed that the relapse rate in TCM group was markedly lower than control group. The levels of serum IL-4 and IL-10 decreased after treatment, and there were significant differences between two groups （P〈0.05）. The levels of serum IL-2 and INF-γ was significantly rose after treatment, and there were significant differences between two groups （P〈0.01）. Conclusion TCM on treating patients with ache vulgaris is effective and safe. It could adjust immune function.
