真皮支架在烧伤创面修复中的影响研究

摘要 目的:探讨负载血管内皮生长因子(VEGF)DNA质粒对胶原-磺化羧甲基壳聚糖真皮支架在修复III度烧伤创面中支架血管化及瘢痕形成的影响,并明确与无支架植入的烧伤创面修复的区别。方法:将负载及不负载VEGFDNA质粒的两种不同胶原-磺化羧甲基壳聚糖真皮支架各移植于6头猪(共12头猪,分别为B和A组)III度烧伤清创后创面,对植入支架1、2、3周后的创面及植入支架1、2、3周创面加植表皮后2周的创面修复(4周)进行观察。同时,通过采用免疫组织化学方法,对不同时间的创面中表达I型胶原和TGF-β1细胞数进行检测和观察。实验以不植入支架的烧伤创面作为对照(C组)。结果:①负载VEGF DNA质粒的胶原-磺化羧甲基壳聚糖真皮支架植入后支架(B组)血管化比胶原-磺化羧甲基壳聚糖真皮支架(A组)要好。②I型胶原表达结果:A、B、C三组I型胶原合成1-3周逐渐增多,4周时A、B组减少,C组增多;1-2周B组最多,C组最少;3周A、B组无显著性差异,C组较少。4周C组最多,B组最少。③TGF-β1表达结果:A、B两组1-2周逐渐增多,至3-4周逐渐下降;C组TGF-β1表达1-3周持续上升,4周时表达减少;1-2周,表达B组最多,C组最少;3-4周,C组表达最多,B组最少。结论:外源性pDNA-VEGF导入人工真皮支架可促进人工真皮更快地血管化的同时,可更好地修复创面和抑制瘢痕增生。
