胃脘止痛汤治疗胃及十二指肠球部溃疡对照观察

Contrast Observation of Gastric and Duodenal Bulb Ulcer Treatment by Using the Epigastric Pain-killer Decoction

[目的]观察胃脘止痛汤治疗胃及十二指肠球部溃疡疗效。[方法]将140例患者分为两组。治疗组110例胃脘止痛汤(党参?25g,黄芪25g,乌药15g,佛手15g,白敛15g,蚕砂15g,黄连15g,水三七10g,吴茱萸8g,内金20g,麦芽15g,丹参10g,郁金10g,白芍15g,公英15g,元胡10g,砂仁15g,参三七20g,白芨15g,鸡子壳15g,乌贼骨20g,九节风15g,佩兰10g,炙甘草10g),随症加减,早晚各150mL。对照组30例奥美拉唑胶囊20mg,早晚各一次,口服。观测临床症状、胃镜、活体组织病理检查、不良反应。治疗1个疗程(30d),判定疗效。[结果]治疗组治愈60例,好转39例,无效11例,总有效率90.00%。对照组治愈9例,好转10例,无效11例,总有效率63.33%。临床疗效、胃镜及活体组织病理检查,治疗组均优于对照组。随访6年,溃疡病复发率治疗组低于对照组。[结论]胃脘止痛汤治疗胃及十二指肠球部溃疡疗效满意,复发率低,优于单纯西药治疗,值得临床推广。

[ Objective ] To observe the curative effect in the treatment of gastric and duodenal Bulb ulcer by using epigastric pain decoction. [ Methods ] 140 cases were divided into two groups in this study. The110 cases in treatment group were treated by Epigastric Pain - killer Decoction （ Dangshen 25g, Huangqi 25g, Wuyao 15g, Foshou 15g, Bailian 15g, Cansha 15g, Huanglian 15g, Shuisanqi 10g, Wuzhuyu 8g, Najin 20g, Maiya 15g, Danshen 10g, Yujin 10g, Baishao 15g, Gongying 15g, Yuanhu 10g, Sharen 15g, Cansanqi 20g, Baiji 15g, Jizike 15 g, Wuzeigu 20g, Jiujiefeng 15 g, Peilan 10g, Zhigancao 10g） , two times a day with 150 ml each time. The other 30 cases in control group were treated by Omeprazole Capsules, two times a day with 20 mg each time. And then we observed the clinical symptoms and the test results of gastroscope and biopsy pathology after one treatment process （30 days）. [ Results ] 60 cases in the treatment group were cured, 39 cases were getting better and 11 cases were invalid with total effective rate of 90.00%. The 9 cases in control group were cured, 10 cases were effective and 11 cases were invalid with the total effective rate of 63.33%. The effect of treatment group was better than that of control group according to the clinical curative effect, gastroscopy and biopsy pathology. The recurrence rate in the treatment group was lower than that of the control group by observation during 6 years. [ Conclusion ] The clinical curative effect by using the Epigastric Pain - killer Decoction was better than that of by using the Omeprazole Capsules with low recurrence rate. Thus, the method used in this study deserves popularization and application.
