恶性肿瘤合并糖尿病患者PICC置管并发症的回顾分析

Retrospective analysis on PICC complications for patients with malignant tumor combined with mellitus diabetes

目的探讨恶性肿瘤合并糖尿病患者PICC置管过程中及导管留置的并发症的发生情况。方法回顾性分析2010年1月至2011年1月42例在化疗过程应用PICC置管的恶性肿瘤合并糖尿病患者的临床资料,统计并分析PICC置管长期留置过程中并发症的发生情况。结果置管过程中存在的问题居前3位的是:导管置入困难,渗血、血肿,导管异位;导管留置并发症居前3位的是:机械性静脉炎、导管相关性感染和导管堵塞。血糖值增高,导管相关性感染及静脉炎发生率明显增加。结论 PICC导管感染与血糖值有关,血糖值增高则导管相关性并发症增多。恶性肿瘤化疗行PICC置管前、化疗后应密切监测血糖。

Objective To explore the complications after PICC indwelling for patients with malignant tumor combined with mellitus diabetes. Methods To retrospectively analyze the clinical data of patients with malignant tumor combined with mellitus diabetes who suffered from PICC during chemotherapy, to observe the incidence of complications during long-term PICC catheter indwelling. Results The top three of catheter problems in the process were： catheter problems, bleeding and hematoma, and catheter ectopic. The top three of complications during long-term PICC catheter indwelling were mechanical phlebitis, catheter-related infections and catheter blockage. The incidence of catheter-related infections and mechanical phlebitis significantly increased with blood glucose values increasing. Conclusions Infection related to PICC catheter is associated with blood glucose levels, and catheter-related complications increase with blood glucose levels increasing. Blood glucose should be closely monitored before and after chemotherapy for PICC catheter indwelling.
