摘要
目的探讨通过缩短起搏器房室间期,阻断心室起搏后室房逆传引起的起搏器介导的心动过速(pacemaker mediated tachycardia,PMT)的可行性及原理。方法回顾分析2009年1月至2011年6月我院起搏器程随访中8例发生PMT患者的资料,PMT均由心室起搏后室房逆传引起,经缩短AV间期处理PMT。结果 8例患者均通过缩短AV间期阻断心室起搏后室房逆传从而终止PMT,最短感知AV间期85ms,最长120ms,平均104±9ms。心电图房室间期(心房感知延迟+感知AV间期)最短120ms,最长150ms,平均136±12ms。结论缩短AV间期能有效阻断心室起搏后的室房逆传,从而消除其介导的PMT。短AV间期未对血流动力学造成负面影响。
Objective To explore the method by shortening atrioventricular delay(AVD) to block the pacemaker mediated tachycardia (PMT). Methods Eight patients were enrolled from January 2009 to June 2011, who had PMT. Results We retrospectively analyzed the materials of 8 patients who was found had PMT during the follow-up, we shortened AVD to terminate the occurrence of PMT, the shortest AVD was 85ms, the longest AVD was 120 (104 ± 9ms). The shortest PR interval in ECG was 120ms, the longest one was 150 (136 ± 12)ms. Conclusion It's a effective method to terminate PMT by shortening AVD in patients with PMT with normal AV interval.
