低压灌注输尿管镜治疗输尿管中、下段小结石71例

The application of low pressure perfusion ureteroscope in the treatment of middle or distal ureteral small calculus:a report of 71 cases

目的:探讨低压灌注输尿管镜治疗输尿管中、下段小结石(直径≤0.6 cm)的疗效。方法:对71例输尿管中、下段小结石行低压灌注下Wolf F8/9.8半硬性输尿管镜治疗。术中18例需联合应用气压弹道碎石。结果:71例均治疗成功,无结石移入肾脏。术中输尿管黏膜撕裂25例,均为合并输尿管中、重度狭窄患者;黏膜下假道2例,均留置双J管引流后治愈;无黏膜撕脱、穿孔等严重并发症发生。结论:采用低压灌注输尿管镜治疗输尿管中、下段小结石效果确切,能有效预防小结石上行移入肾脏。

Objective：To evaluate the therapeutic effects of low pressure perfusion ureteroscope in the treatment of middle or distal ureteral small calculus （ diameter ≤ 0.6 cm）. Methods ： Seventy-one cases with middle or distal ureteral small calculus were treated with low pressure perfusion under a Wolf F8/9.8 semirigid ureteroscope, 18 cases needed to use pneumatic ballistic lithotripsy during operation. Results： All operations were sucessful without any serious complications. Mueosal laceration occurred in 25 cases with moderate or serious ureteral stricture. Two cases with submucosal false passage were cured by detaining double J catheter. No mucosal avulsion and perforation of the serious complications was found. Conclusions ： The effect of low pressure perfusion ureteroseope in the treatment of middle or distal ureteral small calculus is good, which can avoid the migrating of small calculus into the renal pelvis.
