冠状动脉病变与代谢异常的相关性

摘要 目的探讨疑似冠心病(CHD)患者入院后行口服葡萄糖糖耐量试验(OGTT)的必要性,以及糖代谢异常对脂代谢的影响。方法收集郑州市中心医院12个月以来疑似冠心病入住心内科的患者109例,入院后予以OGTT试验、血脂生化及冠状动脉造影检查,其中确诊为冠心病82例,评定Gensini积分,并分析血糖、血脂、冠脉病变之间的关系。结果冠心病组与非冠心病组相比,血脂、(LDL/HDL)、餐后2h血糖升高,其差异有统计学意义,空腹血糖及糖化血红蛋白间差异无统计学意义。冠心病患者血脂(LDL/HDL)与冠脉Gensini积分呈正相关(r=0.576,P<0.01)。结论代谢紊乱是冠心病重要预测因素,冠心病患者多合并血糖血脂代谢异常,其异常程度与冠脉病变程度呈正相关,需早期干预治疗。 Objective To explore the need for admission in patients with to investigate suspected coronary heart disease (CHD) underwent oral glucose tolerance test (OGTT), and abnormal glucose metabolism on lipid metabolism. Methods Chose 12 months since the hospital 109 cases of patients with suspected coronary heart disease, after admission OGTF test, lipid biochemistry and coronary angiography, which diagnosed with coronary heart disease 82 cases to assess the Gensini score and analysis of blood glucose, blood lipids the relationship between coronary artery disease. Results Coronary heart disease group compared with non -CAD group ,blood lipids (LDL/HDL) ,two -hour postprandial blood sugar, the difference was statistically significant, and no significant difference in fasting blood glucose and glycosylated hemoglobin. In patients with coronary heart disease cholesterol(LDL/HDL) and coronary Gensini score was positively correlated ( r = 0. 576,P 〈 0. 01 ). Conclusions Metabolic disorders is an important predictor of coronary heart disease factors in patients with coronary heart disease and more with abnormal blood glucose and lipid metabolism, abnormal degree of coronary artery disease was positively correlated need early intervention.
