肺小腺癌中肺泡细胞癌含量与不同阈值CT测量的磨玻璃密度含量的相关性 被引量:6

Percentage of Lesion with Ground Glass Opacity Evaluated by Ultra-high Resolution CT with Different Thresholds: Correlation with Percentage of Bronchioalveolar Cell Carcinoma Component in Pulmonary Small Adenocarcinoma

摘要 目的:研究肺小腺癌中肺泡细胞癌含量与超高分辨率CT(UHRCT)扫描上磨玻璃密度(GGO)含量的相关性,并筛选出合理的CT测量阈值。方法:最大径≤2cm的肺部孤立性小腺癌52例纳入研究,采用飞利浦64层机完成UHRCT扫描,在标准算法重建1mm层厚肺窗上,分别采用-300Hu、-350Hu、-400Hu、-450Hu、-500Hu和-550Hu6种CT阈值进行病灶GGO含量的定量,同时均完成病理上肺泡细胞癌成分的半定量。两种定量进行Spearman相关性分析。结果:肺小腺癌CT上GGO含量与病理上肺泡细胞癌含量间有明显相关性,当采用阈值为-350Hu进行分析时,其相关性相对更好(r=-0.691,P=0.000)。结论:肺小腺癌中肺泡细胞癌含量与CT上GGO含量具有明显相关性,以阈值-350Hu进行CT定量效果相对更好。 Purpose: To study the correlation between the percentage of bronchioalveolar cell carcinoma component in pulmonary small adenocarcinoma and the percentage of lesion with ground glass opacity (GGO), and to select the reasonable threshold for CT measurement. Methods: Fifty-two patients who had a solitary pulmonary small adenocarcinoma with the largest diameter 〈 2cm were included in the study, all of them were examined on a 64-slice CT (Philips Brilliance 64) with ultra high resolution CT (UHRCT) scanning. The percentage of the lesion with GGO was measured with six CT thresholds (-300 Hu, -350 Hu, -400 Hu, -450 Hu, -500 Huand -550Hu) on the lung window images with high resolution algorithm reconstruction, and the semi-quantitative measurement of the percentage of bronchiolalveolar cell carcinoma component in pulmonary small adenocarcinoma were also done. The correlation between these two kinds of measurements was performed by Spearman correlation analysis. Results: The correlation between the percentage of bronchiolalveolar cell carcinoma component in pulmonary small adenocarcinoma and the percentage of the lesion with GGO was with statistical significant. The correlation (r=-0.691, P=0.000) was better when the CT measurement threshold was set as -350Hu. Conclusion: There is a significant correlation between the oercentage of bronchioalveolar cell carcinoma component in pulmonary small adenocarcinomaand the percentage of the lesion with GGO. The quantitative result is better with the CT threshold set as -350Hu.
