摘要
目的:探讨多层螺旋CT血管成像(MSCTA)评价内脏动脉瘤的价值。方法:回顾性分析18例MSCTA诊断为内脏动脉瘤患者的原始及后处理图像的影像表现。后处理包括最大密度投影(MIP)、容积显示(VR)和曲面重组(CPR)。18例患者中有4例行DSA检查并同期栓塞治疗,5例行手术切除。结果:18例共检出内脏动脉瘤20枚,单发16例,多发2例。其中脾动脉12枚(60%),肾动脉5枚(25%),肠系膜上动脉3枚(15%)。20枚动脉瘤中,发生于动脉起始段7枚,中间段8枚,动脉分叉处5枚。9例经手术或DSA确诊的内脏动脉瘤患者,其MSCTA图像对瘤体部位、大小、形状、瘤壁钙化、附壁血栓及载瘤动脉的显示均与术中所见或DSA一致。结论:MSCTA能清晰地显示内脏动脉瘤及其特征性表现,有助于临床治疗方案的制定。
Purpose: To evaluate the diagnostic value of multi-slice computed tomographic angiography (MSCTA) in visceral artery aneurysms (VAAs). Methods: Eighteen patients with visceral artery aneurysms diagnosed by MSCTA were analyzed retrospectively, of which 4 cases were diagnosed by DSA and 5 cases were confirmed in surgery. The post-processing of CT images included maximum intensity projection (MIP), volume rendering (VR) and curved planar reformation (CPR). Results: Twenty lesions in 18 patients were revealed in MSCTA. Of them, 12 were splenic artery aneurysms (60%), 5 were renal artery aneurysms (25%) and 3 were superior mesenteric artery aneurysms (15%). Seven aneurysms were located at the initial segment of the artery, eight were at the middle segment and five were at the bifurcation. The MSCTA findings of VAAs in nine patients were confirmed by DSA or surgery. Conclusion: MSCT angiography could clearly demonstrated the characteristic findings of VAAs and that would be helpful for the planning of treatment.
