地塞米松对玻璃体切割术后眼内纤维蛋白渗出的影响

Effect of dexamethasone in the vitrectomy in-fusate on experimental postvitrectomy intraocular fibrin

目的 观察地塞米松灌注液对兔眼玻璃体切割术后眼内纤维蛋白渗出的影响。方法 青紫蓝兔 12只 (2 4只眼 )随机分为 3组 ,均进行标准的晶状体粉碎及玻璃体切割 ,术中盲法给予灌注液 ,3组灌注液分别为 :(1)地塞米松组 :平衡盐液中含地塞米松 0 .0 1g· L- 1 ;(2 )低分子量肝素组 :平衡盐液中含低分子量肝素 (速避凝 ) 6× 10 3IU· L- 1 ;(3)对照组 :平衡盐液中不加任何药物。结果 术后地塞米松组眼内纤维蛋白渗出与对照组相比无显著差异 (P>0 .0 5 ) ,仅低分子量肝素组于术后第 1天渗出较轻 ,并有显著性差异 (P<0 .0 0 1)。结论 玻璃体切割术中使用地塞米松灌注液并不能有效减轻玻璃体切割术后的眼内纤维蛋白渗出。

Objective To determine whether dexamethasone(DXM) administered in the infusion fluid has an effect on reducing postoperative fibrin formation in a rabbit lensectomy and vitrectomy model. Methods standard fragmatome lensectomies and core vitrectomies were performed on 24 eyes of 12 rabbits, which were didvided into three groups randomly: the 8 eyes of the first group were treated with 10mg·L -1 of dexamethasone in the infusate; the other 8 eyes in the second group were treated with balanced salt infusion and still another 8 eyes in the last group were treated with 6×10 3U·L -1 of low molecular weight heparine in the infusate and still another 8 eyes in the last group were treated with balanced salt infusion.Results There was no significant difference in postoperative fibrin formation between the eyes treated with 10mg·L -1 of dexamethasone in the infusate and eyes with balanced salt infusion( P >0.05).Only the 6×10 3IU·L -1 low molecular weight heparine infusion resulted in a statistically significant reduction of fibrin formation ( P <0 01) compared with the formers.Conclusion The dexamethasone administered in the infusion fluid does not decrease postoperative fibrin formation in a rabbit lensectomy and vitrectomy model.
