摘要
目的:探讨视觉诱发电位(VEP)在视神经脊髓炎(NMO)和复发性长节段横贯性脊髓炎(r-LETM)诊断中的应用价值。方法:对16例NMO和15例r-LETM住院患者进行VEP检测,并与正常组对照。结果:NMO组:与正常组P100比较,潜伏期显著延长(P<0.05),波幅显著降低(P<0.05);潜伏期异常率高于波幅异常率。r-LETM组:与正常组P100比较,潜伏期差异无显著性(P>0.05),波幅显著降低(P<0.05);波幅异常率高于潜伏期异常率。两疾病组P100比较,NMO组潜伏期显著延长(P<0.05),波幅差异无显著性(P>0.05)。结论:NMO的VEP特点为潜伏期延长为主,伴或不伴波幅下降,严重时波形消失。VEP能够检出r-LETM的亚临床视神经病灶,有助于对NMO的早期诊断。
Objective To investigate the significance of visual evoked potential(VEP) in patients with neuromyelitis optica(NMO) or relapse-longitudinally extensive transverse myelitis(r-LETM).Methods The VEP were examined in 16 inpatients with NMO and 15 inpatients with r-LETM,and the results were compared with those from normal control group.Results The latency of P100 in NMO group was significantly longer than that in control group,and the amplitude was decreased significantly in NMO group(all P 0.05);the abnormal rate of latency was higher than that of amplitude.The P100 amplitude was significantly lower in r-LETM group than that in control group(P 0.05),while no significant difference was found in P100 latency between the two groups(P 0.05);the abnormal rate of amplitude was higher than that of latency.The latency was significantly longer in NMO group than that in r-LETM group(P 0.05),but no significant difference was found in amplitude between the two groups(P 0.05).Conclusions The VEP in NMO was characterized as prolonged latency with or without reduction of amplitude,and wave could disappeared in a badly condition.VEP could help to detect sub-clinical optic nerve damage of r-LETM,which might be helpful in the early diagnosis of NMO.
