CCN1 Changes in the Serum of Patients with Systemic Lupus Erythematosus

摘要 目的通过检测系统性红斑狼疮(SLE)患者血清中CCN1/CYR61(cysteine-rich 61)的水平,探讨其水平变化在SLE、狼疮肾炎发病过程中的作用及与疾病活动的相关性。方法采用酶联免疫吸附法检测SLE患者(n=70)和正常对照组(n=20)血清CCN1水平,收集被检者同批次实验室指标,计算SLE活动指数(SLEDAI),进行统计分析。结果 SLE患者血清CCN1水平高于正常对照组(P<0.05)。SLE肾脏损伤组血清CCN1水平高于肾脏非损伤组(P<0.05);SLE患者血清CCN1水平与抗双链DNA抗体、24 h尿蛋白定量、尿微量白蛋白呈正相关(r=0.507,P<0.05;r=0.552,P<0.05;r=0.506,P<0.05);与补体C3、C4呈负相关(r=-0.591,P<0.05;r=-0.433,P<0.05);与尿常规中高倍镜下红细胞计数、尿蛋白、SLEDAI评分无明显相关性(r=0.185,P>0.05;r=0.265,P>0.05;r=0.199,P>0.05)。结论 SLE患者血清CCN1水平高于正常对照组,与实验室指标相关,推测CCN1可能参与SLE的发病过程,介导局部炎性反应,与淋巴细胞、成纤维细胞异常凋亡有关,但尚不能确定可否作为疾病活动指标。 Objective To evaluate the serum levels of CCN1/CYR61 (cysteine-rich 61 ) in patients with systemic lupus erythematosus (SLE),so as to investigate the correlation CCN1 expression with the pathogenesis and disease activity in SLE patients. Methods The serum levels of CCN1 in rheumatic diseases patients (n =70) with systemic lupus erythematosus (SLE) and normal controls (n =20) were determined by ELISA. The correlation of serum levels of CCN1 with clinical active index was analyzed. Results The serum levels of CCN1 in patients with SLE was higher than normal control group (P 〈 0.05). CCN1 level in SLE patients with kidney damage was higher than the non-kidney damage ones (P 〈 0.05). The sermn levels of CCN1 in SLE were obviously correlated with anti-ds-DNA antibody, 24-hour uri- nary protein excretion,urinary albmnin,C3,C4 (r =0.507,P 〈 0.05;r =0.552,P 〈 0.05;r =0.506,P 〈 0.05;r =-0.591,P 〈 0.05;r =-0.433,P 〈0.05),and there was no correlations with urine RBC count,urine protein,SLEDAI score (r =0.185,P 〉 0.05;r = 0.265, P 〉 0.05;r =0.199, P 〉 0.05 ). Conclusion CCN1 may be involved in the pathogenesis of SLE through inflammatory reaction, ad- justing abnormal cellular apoptosis in lymphocytes and fibmblasts;however,there's still no clear evidence that CCN1 can be used as a disease activity index.
